Florozisli dişlerde sınıf-I kavitelerde kondanse edilebilir kompozit restorasyonların klinik başarısının USPHS Modifiye Ryge kriterlerine göre değerlendirilmesi

Dişsel florozis, diş gelişimi sırasında kronik olarak yüksek seviyede flor alımına bağlı olarak meydana gelen gelişimsel bir problemdir. Birçok olguda diş minesi opak, pöröz, beyazdan sarıkahverengi ve siyaha kadar değişen renklenmiş bir görünüme sahiptir. Isparta, Türkiye’de bulunan endemik bir florozis bölgesidir. Bu çalışmanın amacı, çocuk ve adölesan hastalardaki florozisli daimi büyük azı dişlerine uygulanan bir kondanse edilebilir kompozit rezinin Filtek P60, 3M-ESPE, USA klinik performansının “Modifiye Ryge Kriterleri” doğrultusunda değerlendirilmesidir. ve bütün dişlere Sınıf I restorasyonlar uygulanmıştır. Restorasyonlar 3., 6. ve 12. aylarda, daha önceden kalibre olmuş iki diş hekimi tarafından, kenar renklenmesi, sekonder çürük, kenar uyumu, anatomik form, postoperatif hassasiyet, retansiyon ve renk uyumu kriterleri bakımından değerlendirilmiştir. Dişlerin bir yılın sonundaki son değerlendirilmelerinde, normal, hafif, orta şiddette ve şiddetretansiyon kriterlerinde yüksek oranda başarı gözlenmiştir. Kenar uyumu kriteri için, şiddetli florozis grubunda üç restorasyon “Bravo” skoru almıştır % 11.1 .Bir yıl sonunda Filtek P60 kondanse edilebilir kompozit rezinin klinik performansı oldukça iyi bulunmuştur. Ancak, daha uzun süreli klinik takibin de gerekli olduğu düşünülmektedir

Evaluation of clinical success of condansable composite restorations in class-I cavities in fluorosed teeth according to USPHS Modified Ryge criteria

Dental fluorosis is a developmental problem of teeth, caused by intake of chronical high levels of fluoride during tooth development. In many cases, tooth enamel becomes an opaque, porous and mottled appearance ranging from white to yellow-brown and black. Isparta is an endemic fluorosis area in Turkey. The aim of this study was evaluate the clinical performance of a packable composite resin Filtek P60, 3M-ESPE, USA which applied to fluorosed permanent molar teeth according to “Modified Ryge Criteria” in children and adolescents. Teeth were grouped according to ThylstrupFejerskov Index as; TFI into normal TFI 0 , mild fluorosis TFI 1-2 , moderate fluorosis TFI 3-4 and severe fluorosis TFI 5-7 . 145 Class I restorations were placed in these permanent molar teeth. Restorations were assesed by two previously calibrated operators in 3rd and 6th and 12th month for marginal discoloration, secondary caries, marginal adaptation, anatomic form, postoperative sensitivity, retantion.and colour matching At the final appointment after one year 99.2 %, 99.5 %, 99.1 %, 96.9 % respectively of the restorations in normal, mild, moderate and severe fluorosis groups received Alfa ratings. A high percentage of optimal scores were recorded for secondary caries, anatomic form and retantion. For marginal adaptation, three restorations in severe fluorosis group received a score of Bravo 11.1 % . The clinical performance of Filtek P60 packable composite resin was excellent after 1 year. Nevertheless, we thought that, long-term clinical evaluations are necessary.
