Bruksizmin tanısı, tedavisi ve görüntülenmesi üzerine yeni görüşler

Bruksizm üzerine 1900’lü yÕllardan günümüze kadar yapÕlan birçok çalÕúma olmasÕna ra÷- men bruksizmin etiyolojisi ve patolojisi henüz açÕklÕk kazanmamÕútÕr. Bugün genel olarak kabul edilen görüú uyku bruksizminin morfolojik faktörlerden ziyade çene-yüz fonksiyonlarÕ ve uyku ile ilgili iúlemleri yürüten santral ve otonom sinir sistemindeki bozukluklardan ileri geldi÷i yönündedir. Bruksizmin nedeni tam olarak anlaúÕlamadÕ- ÷Õ için efektif bir tedavi uygulamak ço÷u zaman zor olmaktadÕr. Bu noktada diú hekiminin temel görevi farmakolojik, psikolojik ve dental yönlerden hastanÕn úikayetlerini azaltmak ve çene-yüz bölgesindeki yapÕlarÕ gelecek zararlardan korumaktÕr. AraútÕrmacÕlar úu andaki bilgi düzeyi ile bruksizmin tedavisinde okluzal apareylerle beraber biliúsel-davranÕúsal yöntemlerin en uygun yaklaúÕm oldu÷unu bildirmektedirler. Bu derlemede güncel olarak bruksizm ile ilgili yapÕlmÕú çalÕúmalar ÕúÕ÷Õnda bu kompleks parafonksiyonun nedenleri, görüntülenmesi ve tedavisini de÷erlendirmeyi amaçladÕk.

NEW APSECTS ON DIAGNOSIS, MANAGEMENT AND IMAGING OF BRUXISM

Notwithstanding bruxism is held view in literature since 1900s’ the etiology and pathology of the parafunction is still controversial. Recently bruxism appears to be mainly involved with central and autonomic nerveous system dysfunctions that regulates masticatory system functions more than morphologic factors. An effective threatment is not usually possible because of the unrevealed mechanisms behind the bruxism. In this point the common charge of the dentist is to alleviate complaints of patient and orofacial structures with pharmacological, psychological and dental approaches. The researchers indicate with current knowledge that occlusal appliances and cognitivebehavioral approaches would be convenient in management of the parafunction. In this review we aimed to evaluate the causes, screening and management of bruksizm in the light of actual bruxism studies.
