TÜRKİYE’DE SAĞLIKTA DÖNÜŞÜM PROGRAMININ CEPTEN ÖDEMELERE ETKİSİ: EKONOMETRİK BİR ANALİZ
Bu çalışmada, Türkiye İstatistik Kurumu TÜİK ’dan elde edilen 2004 ve 2016 yıllarına ait hanehalkı bütçe anketi HBA verileri kullanılarak 2016 yılında, 2004 yılına göre sağlık harcaması yapan hanehalkı oranındaki artışı belirleyen nedenlerin, ekonometrik yöntem ile analiz edilmesi amaçlanmaktır. Bu amaçla 2004 ve 2016 yıllarının her biri için ayrı regresyon modeli kurulmuş ve her iki dönemde sağlık harcamasını belirleyen faktörler lojistik tahmin yöntemiyle analiz edilmiştir. Cepten yapılan sağlık harcamalarını etkileyen faktörlerin, söz konusu dönemlere göre farklılaşıp farklılaşmadığı incelenmiştir. Buna ek olarak her iki yıla ait veri seti havuzlanarak Sağlıkta Dönüşüm Programı SDP ’nın sağlık harcaması yapma olasılığı üzerinde yapısal bir dönüşüm yaratıp yaratmadığı incelenmiştir. Ekonometrik tahmin bulguları, SDP’nin cepten ödeme yapma olasılığını etkilediği yönündedir. Ayrıca SDP sonrasında cepten ödeme olasılığını arttıran en temel faktörlerin gelir ve dezavantajlı hanehalkı özellikleri hanede sağlık sorunu olan bireylerin olması, yaşlı ve çocuk bulunması olduğu saptanmıştır.

THE EFFECT OF HEALTH TRANSFORMATION PROGRAM ON OUT OF POCKET PAYMENTS IN TURKEY: AN ECONOMETRIC ANALYSIS
Research Paper This study aims to investigate the determinants of the increase in the rate of households who make out of pocket expenditures oope in 2016, in accordence with 2004 by an econometric analysis. Therefore, the HBS data sets for the year of 2004 and 2016 are used to make a logistic regression analysis to estimate the probability of making out of pocket payment by the households. Two seperate regression models are established both for the years of 2004 and 2016 and the factors which determine the health expenditures are tried to be estimated for both periods. It is examined whether significant variables differ according to the periods examined. In addition, the data set for both years was pooled to see whether the Health Transformation Program HTP created a structural transformation on the probability of spending on health. Estimation findings suggest that HTP affects the possibility of out of pocket payments. Moreover, the most important factors that increase the probability of out-of-pocket payment after HTP are income and disadvantageous household characteristics the presence of individuals with health problems, the presence of elderly and children
