HEMŞİRELERİN ÖNEMLİ İŞLEVLERİNDEN BİRİ: HASTA TESLİMİ

Kaliteli hasta bakımı ve hasta güvenliği çerçevesinde kritik bir öneme sahip olan hasta teslimleri, hemşirelerin her gün vardiya değişimlerinde kullandıkları fonksiyonel bir süreçtir. Hasta teslimlerinde hasta ile ilgili bilgiler sistematik bir süreçte vardiyası sona eren hemşire tarafından yeni vardiyaya başlayacak olan hemşireye iletilmektedir. Hasta teslimlerinin temel amacı, vardiyaya gelen ve giden ekip arasında hasta bakımı doğrultusunda bilgilerin doğru, açık ve sistemli bir şekilde devredilmesidir. Böylece, hasta ile ilgili bilgilerin unutulması ve kaybolması engellenerek advers olayların oranı belirgin düzeyde azalmaktadır. Bu süreçte, hastaların genel durumu gözden geçirilir ve sona eren vardiyada tamamlanmamış görevler belirlenerek bakımın sürekliliği sağlanmış olur. Ayrıca, hasta teslimleri hasta memnuniyetini arttıran faktörlerden biridir.Hasta teslimleri genellikle sözel olarak yapılmaktadır. Bu nedenle hemşireler arasındaki iletişim bu süreçte anahtar role sahiptir. Klinik teslim sırasındaki iletişimsizlik; gereksiz, yanlış, eksik bilgi aktarımına ve zamanın etkin kullanılmamasına neden olabilmektedir. Hasta teslimleri, yatak başı teslim, sözel teslim, yazılı teslim ve kayıtla yapılan teslim olarak sınıflandırılmaktadır. Ayrıca; tüm hasta teslimlerinde hemşirelere rehber oluşturan ve yapılan uygulamaların tek tek gözden geçirilmesini sağlayan hasta teslim modelleri kullanılmaktadır. Bu modellerde kullanılan kontrol listeleri, hasta teslimini yazılı hale getirerek, hasta bakımının dökümante edilmesini sağlamaktadır. Kontrol listelerinin içeriğinde hastanın genel durumu, gözlemler, tedaviler, hasta güvenliğini tehdit eden durumlar gibi bilgiler yer almaktadır.Hastane yönetimleri ve klinikler kendilerine uygun olan hasta teslim modelini seçerek ya da yeni bir model geliştirerek bu sürece dahil olmalıdır. Hemşirelere, hasta tesliminin kendileri ve hastalar açısından öneminin kavranması için eğitimler organize edilmeli ve bu sürecin etkin olarak kullanılması sağlanmalıdır

ONE OF THE IMPORTANT FUNCTIONS OF NUFtSES': HANDOVER

Handovers, which has critical importance at qualified nursing çare and patient safety are functional process' that are used by nurses between shift exchanges every day. Information about patients is transferred systematically by nurses to another nurse during shift exchanges at handovers. The main target of handovers is transferring the information betvveen shift crews correctly, clearly and systematically for nursing. Thus, the information about the patients are not let to be forgotten orgötten lostand thepercentage ofadverse events can be kept at lowlevels. During this process, the general conditions of patients are reviewed and incomplete missions are transferred to next shift. Besides, handovers are one of the increasing factors ofpatient's happiness.Handovers are generally made orally. For that reason, the communication betvveen nurses has a key role on this process. Lack of communication at clinical handover may cause unnecessary and misinformation to be transferred and also can make time not to be used efficiently. Handovers are classified as bedside handover, verbal handover, non-verbal handover and handover with record. Besides, at ali kind of handovers, handover models, as guides for nurses, are used that ensures ali steps are revievved one by one. Checked lists which are used on this handover models make documentation of patient nursing available. Control lists contain information about the general situation of the patient, observations, treatments, situations that threat patient's safety.The hospital managements and clinics have to include themseives to this process by choosing most suitable or by creating their own handover model. Education seminars must be organized for nurses to teach how important handovers are for both patients and themseives. This process must be run efficiently
