Post Sistemlerinde Popüler Yaklaşımlar Ve Fiber Postların Klinik Çalışmaları

Kök kanal tedavisi sonrası restorasyonların uzun dönem başarısı için kalan diş dokusunun korunması, kök çevresinde ferrule etkinin korunması ve uygulanan kök kanal postunun adezyonu en etkili faktörler olarak kabul edilmiştir. Adeziv sistem kullanılarak yapıştırılan fiber postların uzun dönem başarısı, postların kök kırığını azaltan doğal davranışına bağlanmaktadır. Metal postlarda karşılaşılan en yaygın başarısızlık kök kırığı olmasına rağmen fiber postlarda karşılaşılan başarısızlık ise post desimantasyonudur. Fiber postların adezyonunda kök kanalının şekli, post boşluğunun preparasyonu, post translusentliği ve kullanılan adeziv siman tipi endodontik tedaviyle ilgili bir takım faktörlerdir. Fiber post yapıştırılmasında en güvenilir sonuçlar total-etch adezivlerin dual-cure rezin simanlar ile kombinasyonlarından elde edilmektedir. Çalışmalarda fiber postların metal postlar ve tercihen kanal tedavili dişlerde kullanılan zirkon postlar gibi diğer diş renkli postlar için alternatif olarak kullanılabileceğini göstermiştir. Bu derlememizde güncel post sistemlerinden fiberle güçlendirilmiş kompozit postlar, zirkon postlar ve polietilen fiber postların içeriği, kök kanalına ve dentine bağlantısı ve bu postlar ile yapılmış klinik çalışmalar değerlendirilmiştir. Sonuçta bu postların metal postlara alternatif olarak klinikte rutinde kullanılabileceği tavsiye edilmektedir. Uzun süreli klinik çalışmalar bu güncel post sistemlerinin dayanımları hakkında daha iyi bir fikir verecektir

Popular Approaches On Post Systems And Clinical Studies On Fiber Post Systems

Protection of the remaining tooth structure for long term success after root canal treatment, preserving the ferrule effect around the root and adhesion of root canal post are considered as the most important factors. Long term success of fiber posts bonded with adhesive systems is associated with the bio-mimetic nature of posts reducing vertical fractures. While the most encountered reason of failure in metal posts is root fracture, post decementation is also in fiber posts. Root canal morphology, preparation of post space, translucency of post and the adhesive cement type are some of the many factors affecting adhesion of fiber posts. The most reliable results in fiber post cementation are obtained with dual cure resin cements combined with acid-etched, rinsed adhesives. Silanization following silica coating takes a great place in post cementation. It was shown in studies that fiber posts could be used as an alternative to metal posts and zirconia posts used in root canal treated teeth like other tooth coloured posts. Composite posts among contemporary fiber-reinforced post systems, zirconia posts and content of polyethylene fiber posts, their adhesion to root canal and dentine and studies performed about these posts were evaluated in this review
