Tiroid hastalıkları ve bazı ilaçlar inflamatuvar olmayan miyopatiye neden olabilirler. Bu yazıda, tanı konulmamış hipotiroidin eşlik ettiği hiperkolesterolemiyi tedavi etmek amacıyla verilen atorvastatinin indüklediği miyopatili kadın hasta sunulmuştur. Hashimoto tiroiditine bağlı hipotirodili hastada, miyopati atorvastatinin kesilmesinden sonra klinik olarak düzelmiştir. Tiroksin tedavisi ile kas enzimlerinin yüksek seviyeleri normale dönmüştür. Sonuç olarak bu hastada hipotiroidinin atorvastatine bağlı miyopati riskini arttırdığı kanısına varılmıştır. Bu olgu hiperkolesterolemili hastaların, tedaviye başlanmadan önce hiperkolesterolemi nedenleri açısından klinik ve biyokimyasal olarak dikkatli değerlendirilmesi gerektiğini gösterir.

Thyroid diseases and some drugs may cause non-inflammatory myopathy. In this article, a female patient with myopathy induced by atorvastatin given for the treatment of hypercholesterolaemia which is associated with undiagnosed hypothyroidism was presented. Myopathy was clinically improved after cessation of atorvastatin in this patient with hypothyroidism caused by Hashimoto's thyroiditis. High serum levels of the muscle enzymes returned to normal under thyroxin therapy. It is concluded that hypothyroidism increases the risk of myopathy due to atorvastatin. This case highlights the need for careful clinical and biochemical assessment of patients with hypercholesterolaemia for causes of hypercholesterolaemia before initiation of therapy.
