Acil servise başvuran yaşlılarda aneminin değerlendirilmesi

Amaç: Ege Üniversitesi Tıp Fakültesi Acil servisine Aralık 2009-Mayıs 2010 tarihleri arasında başvuran 65 yaş ve üzeri bireylerde anemi sıklığı ile bazı sosyo-demografik ve sağlık özelliklerinin anemiye etkisinin belirlenmesidir. Gereç ve Yöntem: Çalışma kesitsel tipte olup evren; Aralık 2009-Mayıs 2010 tarihleri arasında Ege Üniversitesi Tıp Fakültesi Acil servisine herhangi bir nedenle başvuran 65 yaş ve üzeri 607 birey arasında hematolojik ve biyokimyasal parametreleri bakılan 521 hastayı içermektedir. Bağımlı değişken anemidir. Hemoglobin düzeyi erkeklerde 13 gr/dL, kadınlarda 12.0 gr/dL'nin altında olanlar anemik olarak kabul edilmiştir anemik olanların indeksleri tanımlayıcı olarak değerlendirilmiştir. Bağımsız değişkenler öğrenim durumu, cinsiyet, yalnız yaşama durumu, acile başvuru biçimi (ayaktan/ayaktan değil), böbrek ve karaciğer fonksiyon testleri, açlık kan şekeri düzeyidir. Biyokimyasal ve hematolojik veriler, ortalamaları ve standart sapmaları ile kategorik veriler yüzde dağılımları ile sunulmuştur. Sosyo-demografik ve sağlığa ilişkin değişkenlerin anemi varlığına etkisi ki-kare analizi ile belirlenmiştir. Bulgular: Acile başvuran yaşlılarda anemi sıklığı %63.0'dır. Acile başvuran yaşlıların cinsiyeti çoğunlukla kadın olup (%52.0) yaş ortalaması 76.4±6.7'dir. Ortalama hemoglobin düzeyi 11.8±2.3'tür (kadınlarda 11.5±2.2, erkeklerde 12.1±2.4). Anemisi olanların %76.6'sı normositer, %14.0'ı mikrositer, %9.4'ü makrositer özelliktedir. Erkeklerde kadınlara göre anemi sıklığı anlamlı olarak yüksektir (p=0.007). Assessment of the of anemia in the elderly referred to the emergency unit

Aim: The aim of this study was to determine the frequency of anemia, and the effect of some social, demographic, and healthy features in patients 65 years old and older who referred to the Ege University Emergency Unit (EU) between December 2009 and May 2010. Materials and Methods: This study is cross-sectional study, conducted on 521 patients and which examined laboratory and hematologic data out of 607 elderly &#8805;65 years old referred to the Ege University EU between December 2009 and May 2010. The dependent variable was anemia. Anemia was defined by a decreased hemoglobin concentration (<13 g/dL for men,<12 gr/dL for women). Descriptive data is defined based on the hematological index. Independent variables were education, whether living alone, the manner in which referred to the Ege University EU, kidney and liver function tests, and fasting blood glucose levels. Data were collected by questionnaires which were filled out by trained health providers via face to face interviews. Biochemical and hematological data were present as a mean and standard deviation while categorical data were present as a percentage. Chi-square test was used for determining the effect of social, demographic, healthy features on anemia. Results: The frequency of anemia was found to be 63%. The mean age was 76.4± 6.7, and 52% were female. The mean hemoglobin concentration was 11.8±2.3 gr/dL (11.5±2.2 for women, 12.1±2.4 for men). Patients with anemia had 76.6% normocytic 14%, microcytic 9.4% macrocytic anemia. The anemia frequency was higher in men than women (p=0.007) and also higher in patients with uremia than non uremic patients (p<0.001). Conclusion: The frequency of anemia is high in elderly patients referred to Ege University EU. Anemia is higher in men and it is also higher in patients with uremia.
