Yağ ağrısı ile başvuran ve karaciğerde kist hidatik saptanan çocuk olgu
Dünyada en yaygın görülen insan sestod enfeksiyonu 'Echinococcosis"dir. En sıklıkla karaciğer, akciğer, kemik ve genitoüriner sistemde yerleşerek hastalık tablosu oluştururlar. Onalti yaşında erkek hasta 2-3 gündür olan nefes alırken göğsün sağ alt yarısında batar tarzda ağrı ve sağ yan ağrısı olması nedeniyle başvurdu. Fizik incelemesi normal olan hastanın akciğer grafisinde sağ kostofrenik sinüste kapanmaya yol açari ve sağ diyafragmayı yukarı iten kalsifıkasyon saptandı. Çekilen batın ultrasonografısi ve batın bilgisayarlı tomografisinde, karaciğerde 8. segmentte yerleştiği düşünülen 4,5x6x8 cm boyutlarında, kalın duvarlı yer yer kalsifikasyon gösteren bir kist ile ona komşu yaklaşık 1 cm çapında benzer yapıda küçük bir kist tespit edildi. Bu iki kistik kitle kist hidatik ile uyumlu olarak değerlendirildi. Kist hidatik serolojisi negatif bulundu. Hastaya albendazol (400 mg/g) tedavisi başlandı. Tedavinin üçüncü ayında yapılan incelemelerinde kistin 5x3x2 cm boyutlarına gerilediği görüldü. Yaygın bir enfestasyon olan ve sıklıkla büyük boyutlara ulaşıncaya kadar asemptomatik olarak kalan kist hidatik, yan ağrısı gibi değişik semptomlarla da karşımıza çıkabilmektedir.
A pediatric case with right-sided chest pain and hepatit cyst hydatid
Echinococcosis is the most widespread cestode infestation in the world. The common sites of involvement are the liver, lungs, bone and genitourinary system. Sixteen years old boy presented with right sided chest pain and stabbing pain in the right side of chest while taking breath for 2-3 days. His physical examination was normal. His chest x-ray showed a calcification which caused elevation of diaphragm and the blunting of right costophrenic sinus of the right lung. On the abdominal ultrasonography.and computerized tomography, a cystic lesion which was 4.5x6x8 cm dimensions and has thick wall and calcification area, in the eighth liver segment was detected. Also 1 cm diameter satellite cyst resembling the first cyst was found. The radiological appearance was well-adjusted with the diagnosis of cyst hydatid. The cyst hydatid serology was negative. Albendazole treatment (400 mg/g) was started. The cyst's dimensions were reduced 5x3x2 cm at the three month after the treatment.In conclusion, hydatid cyst is a common infection which can be asymptomatic until it reaches large volumes and it may results different clinical symptoms such as rightsided chest pain.
