Arteria Carotis Externa’nin Lateral Pozisyonu

Arteria carotis externa cartilago thyroidea’nın üst kenarı düzeyinde başlar, hafif bir kavis çizerek yukarı öne doğru ilerler. Arteria carotis externa ve Arteria carotis interna’nın bifurkasyon noktası ile ilgili çok sayıda varyasyon ve anomalilerin olduğu bilinmektedir. Arteria carotis externa’nın lateral pozisyonunun da önceden tanımlandığı gibi nadir olmadığı bildirilmektedir. Dokuz Eylül Üniversitesi Tıp Fakültesi Anatomi Anabilim Dalı laboratuvarında formaldehit ile fikse edilmiş bir erkek kadavrada sağ arteria carotis externa’nın arteria carotis interna’ya göre lateral yerleşim gösterdiği belirlendi. Olguda bifurcatio carotidis, arteria carotis externave dallarının lokalizasyonu, bifurcatio carotidis ile nervus hypoglossus ilişkisi anatomik landmarklara göre değerlendirildi. Değerlendirme için cornu majus’un arka alt ucundan, glandula thyroidea’nın loblarının üst kenarından, ve prominentia laryngea’dan geçen horizontal düzlemler anatomik landmarlar olarak belirlendi. Literatür bilgilerine göre bazı hastalar arteria carotis externa’nın lateral pozisyonu nedeniyle nervus hypogossus basısına bağlı olarak gelişen semptomlarla hekimlere başvurabilmektedir. Bu varyasyonun hastalığın tanısında akla getirilmesi ve bifurcatio carotidis yerleşim bölgesine yapılan cerrahi yaklaşımlarda göznünde bulundurulması önem taşımaktadır.
