Multipl miyelomlu hastalarda polinöropati sıklığı ve laboratuvar parametrelerinin karşılaştırılması
Amaç: Multipl miyelom (MM), kemik iliğinde malign plazma hücrelerinin klonal ekspansiyonu ile karakterize bir B-hücre neoplazisidir. Polinöropati (PN), MM tedavisinin en önemli komplikasyonlarından biridir. Çalışmamızdaki amaç; MMli hastalarda tanı anındaki PN görülme sıklığı ve tedavi öncesi laboratuar parametrelerinin PN açısından belirteç olup olmadığını saptamaktı. Yöntemler: Çalışmaya hematoloji polikliniğinde takipli 31i erkek, 39u kadın olmak üzere toplam 70 olgu retrospektif olarak incelendi. Hastaların tedavi öncesi laboratuar parametreleri ve elektromiyografileri dosyalarından alındı. Bulgular: Olguların tanı anında %15.7inde PN mevcuttu. PNsi olan ve olmayan hastaların β2 mikroglobin, ürik asit, total protein, kalsiyum, ALT, AST, kreatinin, LDH ve albümin değerleri arasında anlamlı farklılık bulunmadı (p>0,05). Sonuç: MMde, PN varlığı veya gelişimini gösterebilen ve erken belirteç olarak kullanılabilecek bir parametre bulunması hastaların takip ve tedavisini kolaylaştıracağı görüldü.
The frequency of polyneuropathy and comparison of laboratory parameters in patients with multiple myeloma
Objective: Multiple myeloma(MM) is a B-cell neoplasia, characterized by clonal expansion of malignant plasma cells in bone marrow. Polyneuropathy (PN) is the most important complications in the treatment of MM. The aim of our study is to find out whether the lab parameters and the incidence of PN, are markers in MM patients or not. Methods: Totally 70 patients (31 male, 39 female) who presented to hematology outpatient department were retrospectively evaluated. Pre-treatment laboratory parameters and electromyography records of the patients taken from the file. Results: 15.7% of the cases the diagnosis was immediately available PN. Levels of β2 microglubin, uric acid, total protein, calcium, ALT, AST, creatinine, LDH and albumin levels didnt differ significantly between two groups (p>0.05). Conclusion: Presence or development of PN in MM and its readiness to be used as an early marker, will obviously facilitate the treatment and follow-up of MM.
