Konjenital ptozisde levator rezeksiyonunda anterior yaklaşım

Levator fonksiyonu 4 mm ve üzerinde olan konjenital ptozisli olgularda anterior yaklaşımla levator rezeksiyonu sonuçları araştırıldı. Çalışmamız, Mart 2001- Mart 2003 tarihleri arasında levator rezeksiyonu uygulanan 12 konjenital ptozisli olgunun 12 gözünü kapsamaktadır. Ameliyat öncesi tüm hastalara görme keskinliği, levator fonksiyon muayeneleri yapıldı. Tüm olgulara anterior yaklaşım ile levator rezeksiyonu uygulandı. Ameliyat sonrasında kapak kenarı düzgün olan ve tam düzelme sağlanan olgular ile . 1 mm rezidüel ptozisi olanlar başarılı olarak değerlendirildi. Olguların levator fonksiyonu 4 ile 10 mm, ptozis derecesi 3 ile 7 mm arasında değişiyordu. Olguların 9’unda (%75) istenen kapak düzeyi elde edilirken 3’ünde (%25) yetersiz düzeltme oldu. 1 olguda açıkta kalma keratopatisi, 1 olguda yetersiz kapak kıvrımı gelişti. Olgular ortalama 12.83 ± 7. 44 (3-24) ay takip edildi. Ptozis cerrahisinde anatomik yapıların daha detaylı görülebilmesi nedeniyle anterior yaklaşımlı levator rezeksiyonu konjenital ptozisde tercih edilebilir bir yöntemdir.

Anterior approach in levator resection in congenital ptosis

Results of levator resection with anterior approach in cases with congenital ptosis with levator function 4 mm or better were evaluated. 12 eyelids of 12 cases with congenital ptosis who had undergone levator resection between March 2001-March 2003 were included in this study. Visual acuity, levator function and grade of ptosis were examined preoperatively. Levator resection with anterior approach was performed in all cases. Full correction or ≤ 1 mm of residual ptosis with a good lid contour is considered successful. Levator function ranged 4 to 10 mm and the amount of ptozis ranged 3 to 7 mm. Nine of the eyelids (75%) were corrected successfully, but 3 eyelids (25 %) were undercorrected. The observed complications were exposure keratopathy in 1 case and abnormal lid crease in 1 case. Mean postoperative follow-up was 12.83 ± 7. 44 monhts (range 3 to 24). Due to better visualization of the anatomical structures, levator resection technique with anterior approach can be preferred in congenital ptosis surgery.
