İn vitro fertilizasyon yapılan hastalarda 3 boyutlu doppler ultrasonografinin implantasyonu belirlemedeki değeri

Amaç: Çalışmanın amacı, invitro fertilizasyon uygulanan hastalarda endometrial ve subendometrial kan akımının gebeliği tahmin etmedeki değerini araştırmaktır. Yöntemler: GnRH agonist long protokol ve antagonist tedavisi uygulanan 2 grup hastalarda oosit toplama gü- nünde vaginal prob ile bilateral uterin arter pulsatilite indeksi ve rezistans indeksi ölçüldükten sonra 3 boyutlu ultrasonografi ile endometrial volüm ölçüldü. Endometrial ve subendometrial kan akım indeksi, damarlanma indeksi ölçülerek her iki grupta da gebelik ile ilişkisi araştırıldı. Gruplar içerisinde gebe olan ve olmayan kişilerdeki doppler ve 3 boyutlu ultrason değerlerinin karşılaştırılmasında t test kullanıldı. P değeri 0.05 altında olması anlamlı olarak kabul edildi. Bulgular: Her iki grupta da gebe olan ve olmayanlar arasında uterin arter ve endometrial-subendometrial akım indeksleri arasında fark bulunmadı (p>0.05). Transfer edilen embriyo sayısında artış gebelik oranlarını yaklaşık 3.5 kat artırmaktaydı (p=0,002). Sonuç: Tek zamanlı ölçülen endometrial ve subendometrial kan akımı ölçümleri implantasyon ya da gebelik başarısını öngörmede anlamlı değildir.

The predictive value of three-dimensional Doppler ultrasonography in determining implantation in patients underwent in vitro fertilization

Objective: The aim of this study is to evaluate the relationship between endometrial and sub-endometrial blood flow and implantation rate in patients whose undergone in vitro fertilization. Methods: A prospective study was conducted. Long protocol and antagonist regimens were administered to the patients. Endometrial and sub-endometrial blood flow was evaluated by using 3 dimensional Doppler ultrasonography on the day of oocyt retrieval measurement. For comparison pregnant and non-pregnant women in terms of endometrial and sub-endometrial blood flow, t test was used. The p value was considered statistically meaningful as <0.05. Results: Endometrial and sub-endometrial blood flow was not different between pregnant and non-pregnant women in both groups (p>0.05). In long protocol group, the number of embryo was greater in pregnant women than non-pregnant women (p=0.012). The number of transferred embryo increased pregnancy rate almost 3.5 fold (p=0.002). Conclusion: The endometrial and subendometrial blood flow is not reliable factor in prediction pregnancy or implantation.
