DENTAL EROZYONUN ETİYOLOJİSİ VE TEDAVİ YAKLAŞIMLARI

Dental erozyon bakteriyel bir etken olmaksızın kimyasal olaylar sonucunda asidin etkisiyle meydana gelen diş sert doku kaybı olarak tanımlanmaktadır. Erozyonun etiyolojisinde iç ve dış kaynaklı olmak üzere birçok etken rol oynamaktadır. Etiyolojinin ortadan kaldırılması erozyonun durdurulmasında önemli rol oynamaktadır. Fakat bunlar mümkün olmadığında florid içerikli vernik, CPP-ACP (kazein fosfopeptit-amorf kalsiyum fosfat kremleri, diş macunu, yüksek florid içeren ağız solüsyonları, lazer ve flor jeli uygulaması gibi koruyucu tedavi yaklaşımları ile diş dokusu güçlendirilebilir. İleri eroziv aşınmalarda restoratif materyal olarak geleneksel tipte cam iyonomer simanlar, rezin modifiye cam iyonomer simanlar, poliasit modifiye kompozit rezinler (kompomer) ve kompozit rezin materyaller tercih edilmektedir. Aşırı ilerlemiş eroziv aşınmalarda ise laminate veneer uygulamaları, kron ya da köprü yapılması gibi restoratif yaklaşımlar tedavi seçeneği olarak değerlendirilebilir. Bu derlemede dental erozyonun etiyolojik faktörleri ve tedavi yöntemleri sunulmuştur.

ETIOLOGY OF DENTAL EROSION AND TREATMENT APPROACHES

In this review, history, action mechanism, Dental erosion is defined as dental hard tissue loss with the effect of acid resulting from any kind of chemical reaction in the absence of bacterial causes. Many factors including intrinsic or extrinsic play role in the etiology of dental erosion. The elimination of etiology plays an important role in the stopping erosion. But when this is not possible, tooth structure can be strengthened with preventive treatment approaches such as application of fluoride-containing varnish, CPP-ACP cream, toothpaste, and mouth rinse containing high-fluoride, laser and fluoride gel. In the excessive tooth wear, the traditional type of glass ionomer cements, resin-modified glass ionomer cements, polyacid modified composite resins (compomer) and a resin composite material is preferred as restorative material. In more excessive tooth wear, restorative approaches such as laminate veneer applications, crowns or bridges can be considered as a treatment option.
