POSTOPERATİF AĞRI KONTROLÜNDE PREEMPTİF TEK DOZ UYGULANAN DEKSKETOPROFEN TROMETAMOL İLE DEKSMEDETOMİDİNİN ETKİNLİKLERİNİN KARŞILAŞTIRILMASI

Amaç: Bu çalışmada; abdominal histerektomi uygulanacak hastalarda postoperatif ağrı tedavisin- de preemptif intravenöz (ıv) deksketorofen trometamol ile deksmedetomidinin analjezik etkileri- ninin karşılaştırılması amaçlanmıştır. Materyal ve Method: Kurumsal onay ve izin alındıktan sonra yaşları 18-65 yaş arasında, ASA I-II,abdominal histerektomi uygulanacak olan 60 hasta randomize olarak 30arlı iki eşit gruba ayrılmış- tır. Hastalara anestezi indüksiyonundan sonra, cerrahi insizyondan 10dk önce deksketoprofen 50mg (Grup I) veya deksmedetomidin 1µg/kg (Group II) iv uygulandı. Postoperatif dönemde morfinkullanılarak Hasta Kontrollü Analjezi (HKA) sağlandı. Hemodinamik parametreler, Visüel AnalogSkala (VAS) ile ağrı ölçümü, sedayon skoru, morfin tüketimi ve olası yan etkiler postoperatif 2, 6,12 ve 24. saatlerde kaydedildi.Bulgular: Postoperatif 24 saatlik dönemde; ağrı skorları VAS ile değerlendirilen gruplar arasındaVAS ağrı skorları benzer olarak bulunmuştur (p>0.05). Toplam 24 saatlik morfin tüketimi Grup I veII için sırasıyla 21,9 ± 13,9 ve 19,6 ± 12,2 mg dı (p>0.05). Yan etkiler açısından da gruplar arasındaanlamlı farklılık bulunmadı (p>0.05).Sonuç: Bu çalışma ile deksketoprofen trometamol ve deksmedetomidininpreemptif analjezik et- kinlikleri benzer bulunmuştur. Aynı zamanda yan etki açısından da deksketoprofen trometamol iledeksmedetomidin arasında anlamlı bir farklılık bulunmamıştır.

Comparison of Efficacy of Single Dose Preemptif Dexketoprofen Trometamol Versus Dexmedetomidine on Postoperative Pain Management

Aim: The aim of the study is to investigate the effects of preemptive intravenous (i.v.) dexketoprofentrometamol and dexmedetomidine on postoperative pain in patients undergoing total abdominalhysterectomy.Material and Methods: After Institutional Approval And Informed Consenthad been obtained,60 patients (between the ages 18-65 yearsand ASA I-II) scheduled for abdominal hysterectomyrandomly allocated into two equal groups. Patients received iv dexketoprofen 50 mg (Group I),iv dexmedetomidine 1µg/kg (Group II) after anesthesia induction and 10 minutes (min) beforesurgical incision. Patient controlled analgesia was supplied postoperatively using morphine.Hemodynamics, visual analogue scale (VAS), sedation score, morphine consumption, and sideeffects were recorded every and at 2, 6, 12 and 24 h after surgery. Results: Pain scores which was measured by VAS score were assessed during the first 24 hafter the surgery. The mean pains cores are similar among groups (p>0.05). The cumulative24-h morphine consumption was 21,9 ± 13,9 and 19,6 ± 12,2 mg for Group I and II respectively(p>0.05). Hemodynamic parameters, sedation scores and side effects did not differ among thegroups (p>0.05). Conclusion: The study demonstrated that preemptive analgesic effects of dexketoprofentrometamol and dexmedetomidine was similar with no signifi cant difference in side effects.
