LİTERATÜR EŞLİĞİNDE 36 FETAL OTOPSİ SONUÇLARININ DEĞERLENDİRİLMESİ: RETROSPEKTİF KLİNİK ÇALIŞMA
Amaç: Perinatal otopsi, termine edilmiş / abort ile sonlanmış fetüslerin veya ölen yenidoğan /infantların anomalilerini saptamak için hala altın standart prosedür olarak görülmektedir. Biz bunedenle fetal otopsi sonuçlarını irdelemeyi ve literatürü gözden geçirmeyi amaçladık.Metod: 2009 ve 2015 yılları arasında İstanbulda özel bir patoloji laboratuvarında incelenmişbiri ikiz gebeliğe ait toplam 36 fetal otopsiye ait patolojik raporlar gözden geçirildi.Sonuç: Bir tanesi ikiz gebeliğe ait olan toplam 36 otopsinin 19’u (52.8%) erkek, 17’si (47.2%)kız fetüs idi. Toplam 35 annenin (1’i ikiz gebelik) en küçüğü 19, en büyüğü 43 yaşında olup,yaş ortalaması 29,35 yıldır. Toplam 36 otopsinin 27’sinde (75%) fetal anomali saptanmamıştır.Geri kalan 9 (25%) olguda ise çeşitli fetal anomaliler tespit edilmiştir. Otopsilere ait plesantalincelemelerde; sadece 2 (5.5%) vakada yaygın plasental hematom ve buna bağlı ölüm tespitedilmiş, 8 (22.2%) vakada plasenta gönderilmemiş, diğer vakalarda ise plasental patoloji saptanmamıştır.Tartışma: Fetal ya da neonatal otopsi, ölüm nedeninin tespit edilmesine, gelişim yaşının belirlenmesine,var ise anomalinin kanıtlanmasına, iatrojenik hastalıkların araştırılmasına ve yenihastalıkların tanımlanmasına ışık tutar ve konjenital anomalileri saptamada ve maternal yenigebeliklere yol gösterici olması açısından otopsi oldukça önemlidir.
Assessment of 36 Fetal Autopsy Results and Review of Literature: Retrospective Clinical Study
Aim: Perinatal autopsy is still seen as the golden standard procedure for detection of abnormalities of terminated / aborted fetuses or dead newborns / infants. We aimed to examine the results of fetal autopsies, and to review the literature. Method: Pathological reports of 36 fetal autopsies, one of which is twin pregnancy, were examined in a private pathology laboratory in Istanbul between the years of 2009 and 2015. Results: A total of 36 autopsies with one twin pregnancy, 19 (52.8%) were male and 17 (47.2%) were female fetuses. The youngest of 35 mothers was 19 years old while the oldest was 43 and their average age was 29.35 years. There were no fetal abnormalities detected in 27 (75%) of the total 36 autopsies. There were fetal abnormalities in the remaining 9 (25%) cases. In the placental examinations of the autopsies, common placental hematoma and related death was detected only in 2 (5.5%) cases, placenta was not sent in 8 (22.2%) cases, and there was no placental pathology in the other cases. Discussion: Fetal or neonatal autopsy sheds light upon the reason of death, determination of developmental age, proof of abnormality if exists, research of iatrogenic diseases, and definition of new diseases. Moreover, autopsy is very important in terms of detection of congenital abnormalities and is a guide for new maternal pregnancies.
