KRONİK DERİN VEN TROMBOZU OLAN HASTADA AKUT GELİŞEN İLİAK VEN TROMBOZUNUN FARMOKOMEKANİK TEDAVİSİ: OLGU SUNUMU
Derin ven trombozu (DVT) derin venöz sistemde bir veya birden fazla venin trombüsle tıkanmasıile gelişip ağrı, şişlik, hassasiyet ve kızarıklık gibi bulgular ile ortaya çıkan tedavi edilmediği taktirdeönemli derecede mortalite ve morbiditeye yol açabilen bir patolojidir. Tedavi seçeneklerinde oralantikoagülan, düşük molekül ağırlıklı heparin (DMAH) ve son dönemde kullanıma giren yeni oralantikoagülan (YOAK) ve farmakomekanik trombektomi işlemi yer almaktadır.Girişimsel tedaviler akut trombüs vakalarında uygulanmakta iken kronik vakalarda etkinlikleri neyazık ki sınırlı kalmakta bu nedenle de kronik olgularda medikal tedavi yine en önemli seçenek olarakkarşımıza çıkmaktadır. Günümüzde artan yaşam beklentisi ve nüks oranlarının sonucu olarakakut ve kronik lezyonların birlikte olduğu vaka sayısı artmıştır.Kombine vakaların tedavisinde farmakomekanik tedavi ne ölçüde kendine yer bulacaktır? Bu makalemizdekronik zeminde popliteal ven trombozu olan bir hastada akut gelişen iliyak ven trombozunafarmakomekanik trombektomi uygulamamız sonrası elde ettiğimiz olumlu sonuçları paylaşmayıamaçladık.
The Pharmacomechanical Treatment of Acute Iliac Vein thrombosis in a Patient Having a History of Chronic Deep Venous Thrombosis
Deep venous thrombosis (DVT) is a pathology occuring as a result of the thrombosis of deep venous system and presenting with pain, edema, tenderness and rash, and has a high risk of morbidity and mortality in case of inadequate treatment. Oral anticoagulant therapy, low molecular weight heparin (LMWH), new oral anticoagulant therapy and pharmacomechanical thrombectomy are the present treatment options for the patients presenting with DVT. Invasive techniques are mostly applied in acute cases of DVT, and medical therapy still remains the primer treatment strategy in chronic cases. In recent years due to increasing rates of predicted survey and recurrent cases acute and chronic forms of DVT have begone to occur simultaneously. What is the place of pharmacomechanical therapy in chronic DVT cases? In this article it is aimed to define the beneficial results of a pharmacomechanical thrombectomy application of a patient presenting with acute iliac vein thrombosis and chronic popliteal vein thrombosis.
