Alkol bağımlılığında panik bozukluğu: retrospektif bir çalışma

Amaç: Panik bozukluğu ve alkol kullanım bozukluklarının birarada görülme sıklığı yüksektir. Bu birliktelik bazı araştırmacı!arca "şelf medikasyon" kavramı ile açıklanmaya çalışılmış, hastaların panik atakların sıklık ve şiddetini azaltmak için alkol kullanmaya başladıkları ileri sürülmüştür. Ancak her iki bozukluğun ortak biyolojik zeminde gelişen ve komorbidite gösteren durumlar olduğu daha çok kabul gören bir kanıdır. Bu araştırmada alkol bağımlılığı ve panik bozukluk tanısı birarada bulunan hastalar incelenmiştir. Yöntem: 1993-2001 yılları arasında Gazi Üniversitesi Tıp Fakültesi Psikiyatri Anabilim Dalı, Alkol Bağımlılığı Ünitesinde yatarak tedavi gören ve alkol bağımlılığı ile panik bozukluğu tanısı birarada konulmuş olan 18 erkek hastanın demografik verileri, hastalık özellikleri, alkol içme şekilleri, anksiyete bulguları ve tedavi sonuçlan incelenmiştir. Bulgular: Hastaların ortalama yaşlan 39.1 (20-54)'dir. Bulgular hastalarda hem alkol bağımlılığı hem de psikiyatrik hastalıklar açısından genetik yüklülüğün yüksek olduğunu göstermektedir. Hastaların %55.6'sı pa-nik semptomlarından önce alkole başlarken (n=10), %44.4'ünde ise önce panik atakları başlamıştır (n=8). Yi-ne hastaların %33.3'ünde alkol kullanımı panik semptomlarından sonra artış gösterirken (n=6), %66.7'sinde panik semptomlarının alkol kullanım miktarı üzerinde etkisi bulunmamıştır. (n=12). Sonuç: Alkol bağımlılarında panik bozukluğu sık görülür. Bulgular, bu durumun bazı hastalarda şelf medikasyon ile açıklanabileceğini ancak diğer birçok hastada her iki bozukluğun ortak bir biyolojik zeminde geliştiğini düşündürmektedir.

Panic disorder in alcohol dependency: A retrospective study

Objective: There is a high rate of comorbidity bet-ween panic disorder and alcohol abuse. Some authors propose that the patients use alcohol as a self-medication to decrease the frequency and intensity of panic attacks. However, it is largely accepted that both disorders share a common biological background. In this study we retrospectively evaluated the patients with dual diagnosis of alcohol dependency and panic disorder. Method: The charts of the patients hospitalized du-ring 1993-2001 in alcohol dependency treatment unit in Gazi University Faculty of Medicine were evaluated. The sociodemographic characteristics, disease features, and treatment response of 18 male patients with dual diagnosis of alcohol dependency and panic disorder were reviewed. Results: The mean age of the patients was 39.1 (20-54). There was a high incidence of family history of psychiatric disorders including alcohol dependency. Ten (55.6%) of the patients reported alcohol abuse prior to panic disorder, while the remaining 8 (44.4%) of the patients experienced panic disorder first. Furthermore, an increase in alcohol abuse after the panic symptoms was observed in 6 (33.3%) of the patients while the rest (66.7%) denied any change in drinking habits. Conclusion: Panic disorder is commonly encountered in alcohol dependent patients. This condition can be explained by self-medication in some of the patients, while in most of the cases these two disorders are thought to originate from a common biological background.
