Türkiye'de streptococcus pyogenes'de makrolid direnç fenotipleri ve direnç genleri (1999-2005)

A grubu beta-hemolitik streptokoklarda eritromisin direnç fenotiplerinin yaygınlığı epidemiyolojik bakımdan önemli olduğu gibi, tedavide doğru antibiyotik seçimini de etkileyen bir faktördür. Bu nedenle ülkemizde 1999-2005 yılları arasında izole edilen ve eritromisine dirençli olduğu saptanan suşların direnç fenotip oranları ve direnç genleri toplu olarak değerlendirilmiştir. Belirtilen tarihler arasında toplam 113 suş incelenmiş ve bunların 66’sı (% 58) M, 37’si (% 33) iMLSB, 9’u (% 8) cMLSB fenotipi olarak belirlenmiş, 1 suş iki fenotip (iMLSB, cMLSB) özelliği göstermiştir. Gen taraması yapıldığında incelenen 48 M fenotipi suşun tümünün mefA; 28 iMLSB fenotipi suşun çoğunun ermB(ermTR); 5 cMLSB fenotipi suştan çoğunun ermB, birinin ermB+mefA, iki fenotip özelliği gösteren 1 suşun yalnız ermB genine sahip olduğu saptanmıştır.

Macrolide resistance phenotypes and resistance genes in streptococcus pyogenes in Turkey (1995-2005)

The prevalence of erythromycin resistance phenotypes in group A beta-haemolytic streptococci is important not only for epidemiology but also for implementing right antibiotic therapy. For this reason, the frequencies of resistance phenotypes and resistance genes in erythromycin-resistant strains isolated in 1999-2005 years in Turkey were evaluated. For this period 113 strains were screened and 66 (58 %) of them were found to be M, 37 (33 %) to be iMLSB, 9 (8 %) to be cMLSB phenotypes. One strain expressed two phenotypic properties (iMLSB, cMLSB). Some strains were investigated for resistance genes and all of investigated 48 M phenotype strains were found to have mefA, most of investigated 28 iMLSB strains were found to have ermB (ermTR) genes. Most of investigated cMLSB strains had ermB, and 1 had ermB+mefA genes. One strains with two phenotypic properties was shown having ermB gene.
