Mitral valv prolapsuslu çocuklarda tanı kriterleri ve izlem

Çalışmamızda mitral valv prolapsus (MVP)'li çocuklarda tanı kriterleri, ekokardiyografinin tanıdaki yeri ve bu hastaların klinik izlemlerindeki önemli noktaların literatürlere gözden geçirilmesi amaçlanmıştır. 1990-2001 yılları arasında Ankara Üniversitesi Tıp Fakültesi Pediatrik Kardiyoloji Kliniği'ne başvuran, kontrollere düzenli gelen ve yeterli verilere ulaşılan 55 hasta başvuru yakınmaları, fizik inceleme ve ekokardiyografi bulguları ile retrospektif olarak değerlendirilmiştir. Çalışmamızda ekokardiyografinin MVP tanısında en önemli metod olduğu, ancak özellikle parasternal uzun eksen ve apikal 4-boşluk pozisyonları olmak üzere birden fazla pozisyonda mitral kapak liflet(ler)inde prolapsus saptanması gerektiği vurgulanmıştır. Ayrıca MVP'li hastaların başvuru sırasında ve takiplerde özellikle mitral yetmezlik ve aritmiler açısından dikkatle değerlendirilmesi ve romatizmal karditli hastaların da MVP açısından takip edilmesi gerektiği vurgulanmıştır.

Diagnostic criteria and follow up in children with mitral valve prolapse

In our study the purpose is to look over the diagnostic criteria, value of echocardiography in diagnosis and important points during clinical follow up of children with mitral valve prolapse (MVP) with the current literature concepts. 55 cases admitted to Ankara University Faculty of Medicine Child Hospital, Department of Pediatric Cardiology between the years of 1990 and 2001 were evaluated retrospectively with their complaints on admission, physical examination and echocardiography findings. It was emphasized that echocardiography was the most important diagnostic method, nevertheless it was necessary to show the prolapse of mitral valve leaflet(s) in more than one echocardiographic view, especially in parasternal long axis and apical 4-chamber positions. It was also emphasized that patients with MVP should be evaluated for mitral regurgitation and arrythmias both on the first admission and during follow up. Patients with rheumatoid carditis should also be evaluated for MVP during follow up period.
