Kronik obstruktif akciğer hastalığında pulmoner hemodinami, solunum fonksiyonları ve hava yolu rezistansı

Pulmoner hipertansiyon ile ventilatuar para¬metreler arası ilişki gündemdeki bir ilgi alanıdır. Ça¬lışmamızda, pulmoner hipertansiyonu bulunmayan (grup 1; n:9) ve bulunan (grup 2; n:17) KOAH'lı ol¬guların pulmoner arter basınçları ile fonksiyonel pa¬rametreleri arasındaki ilişki araştırıldı. Grup I'de hafif hava yolu obstrüksiyonu sap¬tandı. Grup 2'de hava yolu obstrüksiyonu daha be¬lirgindi. Grup Vde hafif dereceli hava yolu rezistans artışı ve grup 2'de ise orta dereceli bir hava yolu re¬zistans artışı vardı ve grup 7 'in değerlerinden anlam¬lı olarak farklı bulundu. Ortalama pulmoner arter basıncı ile ventilatuar parametreler, hava yolu rezis¬tansı ve PaC>2 arasında negatif korelasyon bulundu. Bu bulgular, hava yolu obstrüksiyonu ve hava yolu rezistansı arttıkça pulmoner arter basıncının arttığı hipotezini desteklemektedir.

Pulmonary hemodynamics, pulmonary functions and airway resistance in chronic obstructive pulmonary diseases

The relation between pulmonary hypertension and pulmonary functions is a topic of current inte¬rest. In our trial, the association of pulmonary artery pressures with the pulmonary function tests were se¬arched amongst COPD patients without pulmonary hypertension (group 1) and with pulmonary hyper¬tension (group 2). A mild airflow obstruction was found in group 1. In group 2, the airway obstruction was more pro¬minent. The airway resistance was mildly increased in group I where the airway resistance was modera¬tely elevated in group 2 which was significantly dif¬ferent compared to that of group 1. There were ne¬gative correlations between mean pulmonary artery pressure and ventilatory parameters, airway resistan¬ce and PaOo. These findings support the hypothesis of increased pulmonary artery pressures with more obstructed airways and more increased airway resis¬tance.
