İntrauterin büyüme geriliği respiratuar distres sendromu sıklığını azaltıyor mu?
Amaç: 35 haftanın altındaki intra uterin büyüme geriliği (IUBG) olan pretermlerde respiratuar distres sendromu (RDS) sıklığını belirleyerek IUBG'nin RDS olasılığını azaltıp azaltmadığının incelenmesi. Materyal Metod: Olguların antenatal izlemlerinde anne yaşı, hipertansiyon (HT), sigara içimi, steroid kullanımı, çoğul gebelik öyküsü araştırılmış ve olgular RDS bulguları açısından değerlendirilerek solunum destek tedavisi, sürfaktan ihtiyaçları belirlenmiştir. Bulgular: Çalışmaya IUBG kanıtlanmış, gebelik haftası ortalaması 32.8 (2 hafta ve doğum ağırlıkları ortalaması 1308 (284 g olan 29 SGA ve kontrol grubu olarak benzer gebelik haftasında ve doğum ağırlıkları ortalaması 1751 (441 g olan, 45 gestasyon yaşına uygun doğum ağırlıklı (AGA)preterm alınmıştır. AGA ve SGA'lı bebeklerde anne yaşı, antenatal bakım sayısı ve çoğul gebelik öyküsü açısından anlamlı farklılık saptanmamıştır. Annede HT ve sigara içimi SGA'lı bebeklerde anlamlı olarak daha yüksek bulunmuştur. Her iki grupta solunum sıkıntısı, 7.ve 4. saatde O2 ihtiyacı ve RDS oranında anlamlı farklılık bulunmamıştır. SGA'lı hastalarda solunum sıkıntısı azalmamakla birlikte ventilator gereksinimi (IMV) daha az olmuştur. Exitus oranları incelendiğinde ise SGA'lı hastalarda oranın daha yüksek olduğu görülmüştür. SONUÇ: Intra uterin büyüme geriliği RDS sıklığını azaltmamakta fakat ventilator gereksinimini ve mortalite oranlarını artırmaktadır.
Does intrauterine growth retardation decrease the rate of respiratory distress syndrome?
Objective: To determine the rate of RDS in intrauterine growth-retarded premature infants of less than 35 weeks of gestation. Method: Antenatal factors such as maternal age, hypertension, cigarette smoking, multiple pregnancy and the diagnostic criteria for RDS and sub-sequent surfactant and ventilatory treatment were documented in 74 premature infants who were admitted to the Neonatal Intensive Care Nursery. Results: Twenty-nine small-for-gestational-age infants with an average gestation of 32.8±2 weeks and an average birth weight of 1308±284g were assigned as the study group. Forty-five appropriate-for-gestational-age infants with similar average birth weight and gestational age formed the con-trol group. There were no significant differences between groups with regard to antenatal follow-up, multiple pregnancy and maternal age. Maternal hypertension and cigarette smoking were significantly higher in the study group than in the control group. Respiratory distress, level of oxygen support at the 1st and 4th hour and frequency of RDS were similar in both groups. Though the need for mechanical ventilation was less in the study group, diagnoses of RDS were similar between the groups. Mortality was high in the study group. Comment: Intrauterine growth retardation decreases neither the incidence of RDS nor mor-tality.
