İdiopatik pulmoner hemosiderozis: Bir olgu sunumu ve literatürün gözden geçirilmesi
Yirmi iki yaşındaki erkek hasta kliniğimize ani gelişen halsizlik, çabuk yorulma, şiddetli dispne ve hemoptizi ile başvurdu. İntersitisiyel tipte akciğer infiltrasyonu, hemoptizi, eşlik eden demir eksikliği anemisi ve yapılan bronkoalveolar lavajın da desteklemesi ile idiopatik pulmoner hemosiderozis (İPH) tanısı konuldu ve metil prednizolon ile dramatik cevap alındı. Üç hafta içinde erken nüks gözlenmesi üzerine, önce kortikosteroid sonrasında siklofosfamid uygulandı fakat cevap alınamadı. Hasta masif hemoptizi ve solunum yetmezliği nedeni ile ventilator desteğinde olmasına rağmen kaybedildi. Genellikle çocukluk ve adölesan çağında, nadiren erişkinlerde görülen İPH; demir eksikliği anemisi, hemoptizi ve akciğerlerde yaygın retikulonodüler infiltrasyon ile karakterizedir. Fizyopatolojide tekrarlayan alveolar hemorajiler vardır. Akciğer biyopsisinde alveollerde hemosiderin yüklü makrofajların gösterilmesi ile tanı konulmaktadır. Tedavisinde fikir birliği yoktur ve vakamızda olduğu gibi fatal seyirli olabilir. Ender görülen bir hastalık olmasına rağmen masif hemoptizi, dispne ve demir eksikliği anemisi ile başvuran olgularda akılda tutulması gerekmektedir.
Idiopathic pulmonary hemosiderosis: A case report and literature review
A 22-year-old male was admitted to the hospital because of fatigue, progressive dyspnea and hemoptysis. He had severe iron deficiency anemia and micronodular infiltration of the lung. The presence of hemosiderin within the alveolary macrophages, demonstrated by bronchoscopy and bronchoalveolary lavage, confirmed the diagnosis of idiopathic pulmonary hemosiderosis (IPH). Treatment with corticosteroids was initiated immediately with a dramatic response. However, recurrence was observed within three weeks. Augmentation of immunosuppressive therapy with cyclophosphamide showed no further improvement, and the patient died of massive hemoptysis at the end of the first month following his admittance. Though a rare clinical entity, IPH should be kept in mind in cases of hemoptysis, progressive dyspnea and iron deficiency anemia.
