Demir eksikliği tanısında serum transferrin reseptörü

Kırkdört demir eksikliği anemili hastanın serum transferrin reseptör (sTfR) düzeyi ELISA yöntemi ile ölçülmüş, ortalama değerin 47.85-28.00 nmol/L o/-duğu, normal değerlerden (8.7-28.1 nmol/L) oldukça yüksek olduğu görülmüştür. sTfR konsantrasyonu yaş ve cinse göre değişiklik göstermemektedir. Serum ferritin düzeyi 10 ng/ml'ın üzerinde olan dokuz hastada (%20) sTfR 29.35-9.90 nmol/L gibi yüksektir ve demir eksikliği vardır. Ferritinin sTfR ile korelasyonu bulunmamış, akut faz reaktanı olarak sedimentasyon hızı ile korelasyonu olduğu görülmüştür. sTfR-F indeks (sTfR/log ferritin) hesaplanması, akut enfeksiyon varlığında bile organizma demir metabolizmasının değiştiğini, demir ihtiyacının azaldığını göstermesi açısından yararlıdır. The serum transferrin receptor in the diagnosis iron deficiency

The serum transferrin receptor (sTfR) concent-ration has been measured by an ELISA method in 44 patients with iron deficiency anemia, and the mean level has been found as 47.85-28.00 nmol/L, a value quite above the normal range which is 8.7-28.1 nmol/L. The sTfR concentration does not change with age and sex. in nine (20%) of the 44 patients whose serum ferritin level is above 10 ng/ml, sTfR is high (mean level-standard deviation; 29.35-9.90 nmol/L), and those patients have iron deficiency. The serum ferritin level is not found to correlate with sTfR, but it shows significant positive correlation, like an acute phase reactant, with the erythrocyte sedimentation rate. The calculation of the sTfR-F index (sTfR/log ferritin) is useful in thaf it can show, even in the presence of an acute infection, the change in the iron metabolism of the organism and the reduction in iron need.
