Rokuronyumla sağlanan nöromüsküler bloğa sigara kullanılmasının etkileri

Bu çalışmada; sigara kullanan ve kullanmayan olgularda, rokuronyum ile sağlanan nöromusküler bloğun özelliklerinin karşılaştırılması amaçlandı. ASA I-II gruplarına dahil 40 olgu çalışmaya alındı. Olgular; sigara kullanan (Grup1) (n=20) ve sigara kullanmayan olarak (Grup 2)(n=20) iki gruba ayrıldı. Grup 1'deki olgular adet olarak günde 24 veya daha fazla sigarayı, en az 10 yıl kullananlardan seçildi. Çalışmada TOF uyarısı kullanıldı. İndüksiyon fentanil 2 mgr.kg-1, propofol 2 mg.kg-1 ile sağlandı. 0.6 mg.kg-1 rokuronyum verildikten sonra TOF modunda ilk uyarı yanıtının (T1) kaybolduğu zaman endotrakeal entübasyon uygulandı. İdamede % 1-2 sevofluran, % 50 oksijen- hava karışımı kullanıldı. Ameliyatta 5 dakikada bir verilen TOF uyarısı kullanıldı. Nöromusküler blok etki başlama süresi, klinik etki süresi, derlenme indeksi kaydedildi. Bu değerler Grup 1 ve 2 için sırası ile; 61 ± 5.32 sn, 66 ± 4.12 sn , 9.4 ± 1.48 dk, 14.15 ± 1.5 dk, 8.5 ± 2.1 dk, 14.55 ± 2.18 dk olarak belirlenirken bu değerlerin tümü Grup 1 için istatistiksel anlamlı derecede kısalığı ifade ediyordu. Sonuç olarak, sigara kullanılmasının rokuronyumla sağlanan nöromusküler bloğu etkilediği kanısına vardık.

Effects of smoking on neuromuscular blockade for rocuronium

The purpose of the present study was to investigate the interaction of chronic smoking and the actions of rocuronium. Twenty nonsmokers were compared with 20 smokers who have smoked >24 cigarettes per day for 10 years. Neuromuscular block was monitored by TOF stimulation. Anesthesia was induced with propofol 2 mg.kg-1 iv and fentanyl 2 µg, kg-1 iv. Endotracheal intubation was performed with rocuronium 0.6 mg.kg-1 iv. Anesthesia was maintained with 1-2 % Sevoflurane and 50 % air in oxygen. During the operation, repeated every 5 minute TOF stimulation was used. The mean time of onset in the Group 1 and Group 2 were 61 ± 5.32 sec and 66 ± 4.12 sec respectively (p<0.05). The mean time of clinical duration of action in the Group 1 and Group 2 were 9.4 ± 1.48 min, and 14.15 ± 1.5 min respectively (p<0.05). The mean time of recovery index of neuromuscular blockade in the Group 1 and Group 2 were 8.5 ± 2.1 min, and 14.55 ± 2.18 min respectively (p<0.05). In conclusion, these data reflect the effects of smoking on neuromuscular block by rocuronium.
