Pediyatrik olgularda kaudal blok uygulamasında bupivakain+klonidin ve ropivakain+klonidinin postoperatif analjezik etkileri
Çalışmamızda, kaudal anestezi uygulanan pediyatrik olgularda bupivakain-klonidin ve ropivakain-klonidin kombinasyonlarını, postoperatif analjezi süresi, ek analjezik gereksinimi, idrar yapma süresi ve yan etkiler açısından karşılaştırmayı amaçladık. Fakülte etik kurul onayı alındıktan sonra, ASA I-II risk grubunda alt batın cerrahisi uygulanacak 40 pediyatrik olgu çalışmaya dahil edildi. Premedikasyon uygulanmayan olgular, monitörizasyondan ve sevofluran ile indüksiyondan sonra 2 gruba ayrıldı. Birinci gruba % 0.25 bupivakain ve 2 mg.kg-1 klonidin, ikinci gruba % 0.20 ropivakain ve 2 mg.kg-1 klonidin 1 ml.kg-1 volümde kaudal yoldan verildi. Olgular postoperatif dönemde 24 saat süreyle gözlendi. Rating skalası kullanılarak ağrı, sedasyon skorlaması kullanılarak sedasyon değerlendirmeleri yapıldı. Analjezi süresi, ek analjezik gereksinimi ve ilk idrar yapma süresi saptandı. Bulantı- kusma, motor blok, solunum güçlüğü, bradikardi ve idrar retansiyonu gibi yan etkiler kaydedildi. Gruplar arasında ağrı, sedasyon, motor blok, postoperatif analjezi süresi, analjezik gereksinimi, ilk idrar yapma süresi ve yan etkiler bakımından farklılık bulunmadı. Analjezi süresi 1. grupta 1152.8 dakika, 2. grupta 1212.7 dakika idi. Kaudal analjezide ropivakain-klonidin kombinasyonunun, postoperatif ağrıyı azaltması, motor blok görülmemesi, idrar retansiyonuna yol açmaması nedeniyle,bupivakain-klonidin kombinasyonuna alternatif olarak kullanılabileceği kanısındayız.
The effects of caudal bupivacaine+clonidine and ropivacaine+clonidine combinations on postoperative analgesia in paediatric patients
In this study it was aimed to compare the duration of analgesia, need for additional analgesic agent, first urination time and side effects of bupivacaine-clonidine and ropivacaine-clonidine combinations for caudal analgesia in paediatric patients. After approval of the ethic committee 40 ASA I-II paediatric patients undergoing lower abdominal surgery were allocated randomly to receive a caudal injection of either a mixture of 0.25 % bupivacaine with 2 mg.kg-1 clonidine (Group I) or mixture of 0.2 % ropivacaine with 2 mg.kg-1 clonidine (Group II). Patients were not premedicated. Anaesthesia was induced with sevoflurane in decreasing concentrations. Patients were observed for pain (by rating scale), sedation (by sedation score) and motor block (by Bromage scale) for 24 hours. Duration of analgesia, and additional analgesic requirement were detected. Side effects (nausea-vomiting, respiratory depression, bradicardia, and urinary retention) were noted.. There was no difference in pain, sedation, first urination time, motor block scores, duration of postoperative analgesia, additional analgesic requirement, and side effects between groups. The duration of analgesia was 1152.8 minutes in Group I and 1212.7 minutes in Group II. We conclude that, because of the similar postoperative analgesic effect, less motor blockade, and less urinary retention, ropivacaine-clonidine combination can be an alternative to bupivacaine-clonidine combination in caudal analgesia.
