ANESTEZİ UZMANLARININ ZOR HAVAYOLU YÖNETİMİNE YAKLAŞIMLARI
Amaç: Anestezistlerin temel görevlerinden olan havayolu yönetimi için farklı preoperatif değerlendirme yöntemlerinin bilinmesi,yeni teknolojik araçların kullanımının öğrenilmesi ve aralıklarla eğitim alınması önemlidir. Çalışmadaki amacımız bölgemizdekianestezi uzmanlarının zor havayoluna yaklaşımlarını araştırmak, eksiklikleri saptayarak yeni eğitim programlarının hazırlanmasıiçin önerilerde bulunmak.Yöntem: Zor havayolu için; preoperatif değerlendirme yöntemleri, tercih ettikleri aletler ve anestezi yöntemleri, zor havayoluhakkında aldıkları eğitimler ve kullandıkları algoritma varlığı sorularını içeren anket formu hazırlandı. Sorular çoktan seçmeliydi.Etik kurul onamı alındıktan sonra anket bölgemizdeki anestezistlere uygulandı.Bulgular: Ankete katılan 60 anestezistin uzmanlık süresi ortalama 12± yıl idi. Preoperatif havayolu değerlendirmesi yapanlarınoranı %98,3 idi. "Zor havayolu ile karşılaştığınızda ilk seçeneğiniz nedir?" sorusuna yardım çağırmak (%46) ve supraglottikhavayolu gereçlerini kullanmak (%40) cevabı verildi. Zor entübasyonda daha etkin kullanılan alet sorusuna Fastrach yanıtı verildi(%63,3). Mezuniyet sonrası zor havayolu ile ilgili bir eğitim programına katılanların oranı %43,3 idi. Katılanların %51,7'sininçalıştığı kurumda zor havayolu ile ilgili bir algoritması vardı. Sonuç: Araştırmanın sonucunda preoperatif değerlendirmenin öneminin bilindiği, ancak kullanılan aletler ve algoritmakullanımı konusunda bilgi eksikliği olduğu tespit edildi. THE APPROACHES OF THE ANESTHESIOLOGISTS TO DIFFICULT AIRWAY
Objective: In preoperative assessment for airway management which is one of the main tasks of anesthesiologists, learningthe usage of technologica linnovation and receiving training in intervals are important. The aim of our study is to determineapproaches to difficult airway of the anesthesiologists working in local region and to advise for preparation of new educationprogrammes by detecting the deficiency.Method: For difficult airway; a questionnaire was formed including the methods of preoperative assessment, the devices theyprefer and anesthesia methods, trainings about difficult airway and the presence of an algorythm. The questions are multiple-choice.After the approval of ethics commitee, the questionnaire was delivered to the anesthesiologists in our region. Results: The mean duration of expertise of the surveyed 60 anesthesiologist was 12 years. The proportion of the anesthesiologistsperform preoperative assessment was 98,3 %. "What is your first choice when you encounter with difficult airway?" was asked,46 % call for help, 40 % using supraglottic airway devices answers were taken. To the question about more efficient device fordifficult airway, Fastrach reply was given (63,3 %). The ratio of the anesthesiologists joined to a training about difficult airwaywas 43,3 %. 51,7 % of the participants had an algorythm in their institutions.Conclusion: In conclusion of the study it is established that the importance of preoperative evaluation is well known, but thereis a lack of knowledge about the usage of tools and the algorithm.
