Akut akciğer hasarında intratrakeal deksametazonun hemodinami ve arter kan gazlarına etkileri
Amacımız, tavşanlarda hidroklorik asit (HCl) aspirasyonu sonucu gelişen akut akciğer hasarında (ALI), intratrakeal (İT) uygulanan deksametazonun hemodinami ve arter kan gazlarına olan etkilerini araştırmaktır. Çalışmaya alınan 14 tavşan, kontrol grubu (KG, n=6) ve deksametazon grubu (DG, n=8) olmak üzere rasgele iki gruba ayrıldı. Hayvanlara, ketamin hidroklorür 50 mg kg-1 im uygulanarak anestezi sağlandı ve trakeostomi açıldı. A. aurikularise 24 G iv kanül yerleştirilerek 30 dk aralıklarla ortalama arter basınçları (OAB), kalp atım hızları (KAH) kaydedildi. Hayvanlar, basınç kontrole madunda FiO2: 1.0, f: 80 dk-1, VT:10 mL kg-1, İ/E: 1/2, PEEP: 5 cmH2O parametreleriyle 5 saat ventile edildiler. Başlangıç arter kan gazı örneği alındıktan sonra tüm tavşanlara 2 mL kg-1 HCl İT verildi. DG'na HCl verilmesinden 5 dk sonra 2 mL içinde deksametazon 2.5 mg kg-1, İT verildi. The effects of intratracheal dexamethasone on hemodynamic and arterial blood gases in acute lung injury
The aim of this study was to demonstrate the effects of intratracheal dexamethasone on hemodynamics and arterial blood gases in hydrochloric acid (HCl) aspiration induced acute lung injury of rabbits. Fourteen rabbits were randomly divided into two groups, as the control (CG, n-6) and the dexamethasone groups (DG, n=8). An endotracheal tube was placed through a tracheostomy in all animals following ketamine hydrochloride (50 mg/kg<sup>-1</sup> IM). 24 G iv cannula was inserted to a. auricularis then the mean blood pressure (MAP) and heart rate (HR) were recorded 30 at min. intervals. The animals were ventilated with pressure control mode for 5 hours with the parameters FiO<sub>2</sub>:1.0, RR: 80 min<sup>-1</sup> V<sub>T</sub>: 10mL kg<sup>-1</sup>, HE: 1/2, PEEP: 5 cmH<sub>2</sub>O. Intratracheal HCl (2 mL kg<sup>-1</sup>) was given following the initial sampling of arterial blood gases. Five minutes after the application of HCl the DG received IT dexamethasone (2.5 mg kg<sup>-1</sup>). PaO<sub>2</sub>, PaCO<sub>2</sub> values were recorded at the second and fifth hour of HCl aspiration. MAP was much more significantly decreased in 240th, 270th and 300th min, in the CG (p<0.001). MAP did not significantly change in the DG. HR was significanty decreased 210th, 240th, 270th and 300th min. in the CG, (p<0.05). HR did not change significantly in the DG. In both groups PaO<sub>2</sub> was found to be decreased when compared to initial PaO<sub>2</sub>, but reduction in PaO<sub>2</sub> was significant in the CG (p<0.05). PaCO<sub>2</sub> was significantly increased in the CG (p<0.05). PaCO<sub>2</sub> was not significantly changed in the DG. In conclusion; IT dexamethasone given after 5 minutes of HCl aspiration had beneficial effects on hemodynamic parameters and arterial blood gases values.
