ÇOCUKLUK ÇAĞI ADLİ OLGULARININ DEĞERLENDİRİLMESİ VE KORUYUCU ÖNLEMLER
Amaç: Harici etkenler sonucu oluşan, kişilerin beden-ruh sağlığının bozulmasına ya da ölümüne sebebiyet veren olaylar tıpta 'adli olgu' kavramını oluştururlar. 18 yaş altı çocukluk yaş grubunda adli nitelikteki olgulara sıklıkla rastlanılmaktadır. Bu çalışmada çocukluk yaş grubu adli vakaların özellikleri incelenerek önleyici yöntemlerin tartışılması amaçlandı. Yöntemler: Ocak 2008 - Şubat 2012 tarihleri arasında Mustafa Kemal Üniversitesi Tıp Fakültesi Acil Servisine başvuran 68498 vaka içerisinde yer alan adli olgular retrospektif olarak incelendi. Olgular yaş, cinsiyet, hastaneye geliş şekli, başvuru ayları, olayın mekanizması ve klinik özellikleri açısından değerlendirildi. Bulgular: Ocak 2008-Şubat 2012 tarihleri arasında acil servise baş- vuran tüm adli olguların sayısı 1093 olup bunların 237(%21)'si çocukluk yaş grubunda idi. Bu olguların 143(%60,3)'ü erkek, 94(%39,7)'ü kız cinsiyetinde olup, ortalama yaşlar erkeklerde ve kadınlarda sı- rasıyla 9.95±5,49(1-18yaş) ve 10,72±6,74(1-18yaş) idi. Olguların en sık 13-18 yaş grubunda (%45,9) olduğu; hastaneye en sık başvuru nedenlerinin erkeklerde trafik kazası (%33,6), kızlarda ise zehirlenme (%42,6) olduğu; olguların %66'sının hastaneye kendi imkânlarıyla geldiği, en çok başvurunun (%10.9, n=26) ağustos ve ekim aylarında olduğu, olguların %84,3 (n=200)'ünün hayati tehlike oluşturmayan yaralanmalar olduğu, %50,2(n=119)'sinin acil gözlemde takip sonrası taburcu edildiği, %45,9(n=109)'unun ise yatarak tedavi sonrası ilaçlarla taburcu edildiği, %2,9(n=7)'unun olgunun acil serviste öldüğü belirlendi. Sonuç: Adli olguların önemli bir kısmını da çocukluk çağı olguları oluşturmaktadır. Çalışmamızda en sık 13-18 yaş grubu çocukların adli olgu olarak hastaneye başvurması dikkat çekici bulunmuştur. Olguların önemli bir kısmının trafik kazası ve zehirlenme nedeniyle acil servise başvurduğu görülmüş olup bu olayların önlenmesine yönelik hukuki ve sosyal çalışmalar yapılması
EVALUATION OF CHILDHOOD FORENSIC CASES AND PROTECTIVE MEASURES
Objective: Incidents that occur as a result of external factors and cause deterioration of individuals' corporalspiritual health or death constitute the concept of 'forensic case' in medicine. Under the age of 18 in childhood age group forensic cases are frequently observed. It is aimed to analyze the features of the childhood age group forensic cases and discuss preventive methods in this study. Methods: The forensic cases among total 68498 cases referred to the Emergency Service of Mustafa Kemal University Medical Faculty between January 2008 and February 2012 were analyzed retrospectively. The cases were evaluated in terms of age, sex, the way of arrival, the month of referral, mechanism of event and clinical features. Results: The number of all forensic cases between January 2008 and February 2012 were 1093 and 237 (21.0%) of them were in childhood age group. One hundred fortythree of 237 cases were males (60.3%), 94 were females (39.7%) and the average age of males and females were 9.95±5.49 (1- 18 years), and 10.72±6.74 (1-18 years) respectively. It has been determined that most of the cases (45.9%) were in the 13-18 age group; most frequent causes of referral were traffic accident in men (33.6%) and intoxication in women (42.6%); 66% of the cases had come to the hospital at their own expense; most of the referrals (10.9%, n=26) to hospital were in August and in September; 84.3% (n=200) of the cases were reported to be non life-threatening injuries; 50.2% (n=119) of the cases were discharged from the hospital after follow-ups in the observation unit of the emergency service, 45.9% (n=109) of the cases were discharged with medications after being hospitalized and 2.9% (n=7) of the cases were died in the emergency service. Conclusion: Childhood forensic cases consititute an important part of all forensic cases. In this study, 13 to 18 childhood age period was remarkably frequent in forensic cases. It is considered that legal and social studies should be performed to prevent traffic accident and intoxication cases which are the most frequent causes of forensic cases.
