Ankara'da yabancı cisim aspirasyonu sonucu ölen olguların değerlendirilmesi

Yabancı cisim aspirasyonu, özellikle çocuklarda ve yaşlılarda iyi tanımlanmış bir sağlık problemidir. Bu çalışmada, 1999-2003 yılları arasında Ankara'da otopsisi yapılan ve yabancı cisim aspirasyonu sonucu öldüğü tespit edilen 8 olguya ait veriler sunulmuştur. Olgulardan 2'si 4 yaş altındadır. 2 olguda psikiyatrik hastalık hikayesi alınırken, 1 olguda çene-boyun bölgesine yönelik geçirilmiş çok sayıda operasyon ve diş protezi, 1 olguda ise zihinsel gelişme geriliği hikayesi alınmıştır. 7 olguda solunum yolunu farklı lokalizasyonlarda ve derecelerde tıkayan yabancı cisim tespit edilmiş, ihmal ve istismar şüphesi uyandıran cilt bulguları olan ve zihinsel gelişme geriliği bulunan olguda ise yabancı cisim bulunamamış, akciğer histopatolojisinde ödem saptanmıştır. Destekleyici klinik bulgu yada görgü tanığı olmadan agonal ve postmortem dönemdeki geçişleri gerçek aspirasyondan ayırmanın zor, bazen imkansız olduğu bilinmeli ve makroskopik bulgular histopatolojik incelemeyle desteklenmelidir.

Evaluation of deaths by foreign body aspiration in Ankara

Foreign body aspiration is a well defined health problem especially in children and elderly. The autopsy results of 8 cases of deaths by foreign body aspiration in Ankara between 1999-2003 were presented. Amongst the cases; two were younger than 4 years old, two had psychiatric disorder history, whereas one had history of sevaral chin and neck surgeries and dental prothesis and the other had mental retardation history. In 7 cases, foreign bodies were determined in various localizations of the respiratory tract. In the last case, with mental retardation history, showed skin lesions of abuse and neglicence suspicion, there was no foreign body in respiratory tract, however edema was determined in lung histopathology. It is difficult to distinguish the transition during agonal and postmortem period from the actual aspirations without any supporting clinical finding or eyewitness, and the macroscopic findings might be supported with histopathological evaluation.
