Çoklu Epidural Hematomlar: Sistematik Literatür Taraması ve Nadir Bir Olgu Sunumu

GİRİŞ ve AMAÇ: Bu çalışmanın amacı literatür ışığı altında çoklu epidural hematomların ve klinik özelliklerinin gözden geçirilmesi ve tartışılmasıdır. YÖNTEM ve GEREÇLER: Bu çalışmada tıbbi literatürde yayınlanan çoklu epidural hematom olguları toplandı ve kliniğimizden yayınlanmamış bir olgu eklendi. BULGULAR: Yaptığımız literatür taramasında arama kriterlerimizi sağlayan 50 adet makale bulundu. Bunların 38 tanesi tekli olgu sunumları iken, 12 makale çoklu olgu sunumları ve klinik serilerden oluşmaktaydı. Olguların büyük çoğunluğunda bilateral epidural hematom saptandı. Sadece 7 olguda ikiden fazla epidural hematom tespit edildi. İki olgu dışında tüm olgularda cerrahi tedavinin uygulandığı görüldü. TARTIŞMA ve SONUÇ: İntrakraniyal epidural hematomlar genellikle kafa travmasına sekonder gelişirler ve tek taraflı oluşurlar. Çokluepidural hematomlar oldukça nadir olgulardır ve prognozları genellikle operasyon öncesi nörolojik durum ile bağlantılıdır.

Multiple Epidural Hematomas: Systematic Lıterature Review and an Unusual Case Presentation

INTRODUCTION: The aim of this study was to review and discuss the clinical properties of multiple epidural hematomas in light of the literature. METHODS: In this study, we pooled cases with multipl epidural hematomas that were published in the medical literature and added an unpublished case from our institution. RESULTS: Fifty published articles which are providing our criteria was found in our literature review. While thirty-eight of them were single case reports, twelwe were multipl cases and clinical series. Most of cases have bilateral hematomas. More than two epidural hematomas were found in only 7 cases. Except two cases, surgical treatment was used in all patients. DISCUSSION AND CONCLUSION: Intracranial epidural hematomas are generally secondary to head traumas and occur unilaterally. Multiple epidural hematomas are very rare condition and prognosis is mainly dependent of the preoperative neurological state.
