Posterior Üretral Valvli OlgulardaAilesel İnsidens
Giriş ve Amaç: Ailesel posterior üretral valv ile ilgili literatürde az sayıda yayın bildirilmiştir. Posterior uretral valv etyolojisinde genetik faktörlerin rolü olduğu bazı çalışmalarda vurgulanmıştır, ancak belirli bir gen defekti için halen net bir kanıt gösterilememiştir. Prenatal tanısı nedeniyle takibimizde olan olgunun ablazyon işlemi tamamlandığında, ziyarete gelen kardeşinde karın şişkinliğinin fark edilmesi sonrası yapılan fetkikierde tanılanan kardeş postreior üretral valv olguları sunulmuştur. Olgu 1: Prenatal ultrasonografide bilateral hidroüreteronefroz saptanan hastada postnatal çekilen voiding sistoüretrografide bilateral Grade IV- V vezikoüreteral rejlü ve posterior üretra geniş olarak izlendi. Vezikostomi sonrası bir yaşında yapılan sistoskopide Tipi valv nedeniyle ablazyon yapıldı. Vezikostomisi kapatıldıktan sonra temiz aralıklı kateterizasyon (TAK) programına alındı. 3 yaşında çekilen voiding sistoüretrografide rejlü saptanması üzerine bilateral Lich-Gregoir antirejlü operasyonu yapıldı, hasta halen takiptedir. Olgu 2: Kardeşinin posterior üretral valvi abiaze edildiğinde onu ziyarete gelen 4 yaşındaki ağabeyinde ileri derecede batın distasiyonu olması nedeniyle yapılan ultrasonografide bilateral hidroüreteronefroz saptandı. Voiding sistoüretrografide bilateral Grade V rejlü Görüldü. Sistoskopide Tip I valv görülerek abiaze edildi ve TAK programına alındı, ancak üretrada yalancı yol oluşması dolayısıyla kateterize edilemedi. Mesanenin boş altılamaması nedeniyle vezikostomi açıldı. 2. yılında çekilen voiding sistoüretrografide rejlünün devam etmesi üzerine bilateral Lich- Gregoir antirejlü operasyonu yapıldı. Hasta halen takiptedir. Sonuç: Her iki kardeşte TAK işlemine rağmen ileri derecede hidrone.frozun daha da artarak devam etmesinin nedeni, annenin TAK işlemini her iki çocuğuna etkin bir şekilde uygu/ayamamasından kaynaklanmasıydı. PUV tanısı alan her olgunun ailesinde erken tanı için üriner ulrasonografi taraması yapılmasını önermekteyiz. Posterior üretral va/v/erin genetik etiyolojisini tanımlamak için prospektifve retrospektifdaha fazla çalışmaya ihtiyaç vardır.
Familial ineidence in cases of the posterior urethral valves
Introduction and Aim: Previously, it has been reported a possible role ofgenetic factors in the etiology of posterior uretral valv (PUV). However, no c/ear evidence for a specific gene defect in PUV has been detected There are only a few reports in the literature on familial posterior urethral valve cases. Here we present two brothers with PUV, one of them get diagnosed incidentally. Cas e 1: Bilateral hydroureteronephrosis was diagnosed prenatally in younger brother. Postnatal voiding cystouretrography (VCUG) revealed Grade V bilateral vesicoureteral re.flux (VUR). Vesicostomy was performed Ablasion was performed at one year of age for Type I valve. After repair of vesicostomy, he admitted to clean intermittant cateterization (CIC) program with anti-cholinergic m edication. Case 2: The diagnosis of bilateral hydroureteronephrosis was made incidentally in 4-year-old boy, when he came to visit his operated brother. At the time, he had an noticeab/e abdominal distention. Abdominal ultrasonography revealed bilateral hydroureteronephrosis, VCUG showed bilateral Grade V VUR. On cystoscopy Type I valve identified and.fulgurated CIC program was started with anti-cholinergic medication. However, it was noticed the emptying of bladder is not appopriate due to false urethral passage. Vesicostomy was performed to emptying the bladder. Conclusions: In familial cases of PUV,.further and detailed investigations are needed to define of a possible genetic etiology. We suggest screening in siblings of patients with posterior urethral valves in order to achive early diagnosis and optimal care.
