Prostat manyetik rezonans - ultrasonografi görüntülüme füzyon biyopsinin etkinliği, ilk deneyimimiz

Amaç: Prostat kanseri erkeklerde çok sık görülen bir kanser türüdür. Tanı amacıyla, tümör görüntüleme duyarlılığı yüksek olmayan transrektal ultrasonografi US eşliğinde biyopsi kullanılmaktadır. Bu çalışmada manyetik rezonans MR görüntülemeyle ortaya konulabilen kuşklu lezyonlardan çok yeni bir yöntem olan MR-US füzyon biyopsi yöntemiyle yapılacak biyopsi sonuçlarının duyarlılığını ve hasta yönetimine katkısını tartışmayı amaçladık.Gereç ve Yöntemler: Hastanemiz üroloji polikliniğine başvuran, klinik ve/veya laboratuar bulguları sonucu prostat kanseri şüphesi olan hastalar multiparametrik prostat MR çekimi için radyoloji kliniğimize yönlendirildi. MR’ da PIRADS sınıflamasına göre 3,4 ve 5 lezyon tanısı alan hastalara MR-US füzyon biyopsi işlemi yapıldı. Daha önce standart biyopsisi olmayan hastalara 12 kor standart biyopsi işlemi de füzyon biyopsiye ek olarak gerçekleştirildi.Bulgular: Çalışmaya 15 hasta dahil edilmiştir yaş aralığı 45-71, ortalama 53 . Toplam 2 adet PIRADS 3, 8 adet PIRADS 4 ve 5 adet PIRADS 5 lezyon MR ‘da izlenmiştir. Histopatolojik değerlendirme sonucunda bir hastada PIRADS 3 tanılı lezyondan füzyon biyopside benign prostat dokusu sonucu alınmasına rağmen standart biyopsi ile Gleason 3+3 prostat kanseri tanısı alınırken bir hastada da PIRADS 4 tanılı lezyondan füzyon ve standart biyopsi sonucunda benign prostat dokuları gelmiştir. Bir vakada ise PIRADS 4 lezyondan füzyon biyopsi sonucu benign prostat dokusu gelmiştir. Diğer vakaların hepsinde Gleason 3+3, 3+4 ve 4+3 prostat kanseri sonuçları alınmıştır.Sonuç: Füzyon biyopsi işleminin standart biyopsi işlemindeki potansiyel örnekleme hatalarını elimine ederek hastaların daha doğru tanı almasına katkı sağlayabileceğini ve böylece prostat kanserli hastaların yönetiminin iyileştirilmesine olumlu katkı sağlayacağını düşünüyoruz

The effectiveness of prostate magnetic resonance-ultrasound imaging fusion biopsy, our first experience

Objective: Prostate cancer is frequently encountered in men. Trans-rectal ultrasound guided biopsy which has low tumor detection sensitivity, is implemented in order to call diagnosis. In this study we aimed to evaluate the sensitivity of magnetic resonance-ultrasound MR-US fusion biopsy in the diagnosis of MR suspect lesions and the contribution of this new method to the patient management.Materials and Methods: Patients who were suspicious of having prostate cancer were sent from urology department to radiology department to perform multi-parametric prostate MR imaging. MR-US fusion biopsies were performed to patients who had PIARDS 3,4 and 5 lesion in MR imaging. 12 core standard biopsies were also performed after fusion biopsy if the patient did not have prior standard biopsy.Results: 15 patients were included in our study 45-71 years, mean 53 . MR depicted 2 PIRADS 3, 8 PIRADS 4 and 5 PIRADS 5 lesions. In one patient with PIRADS 3 lesion, fusion biopsy result was benign prostatic tissues but standard biopsy result was Gleason 3+3 lesion, another patient with PIRADS 4 lesion both fusion and standard biopsies were benign prostatic tissues. A different patient with PIRADS 4 lesion fusion biopsy was benign after histopathologic evaluation. All other patient’s pathologic results were Gleason 3+3, 3+4 and 4+3 prostate cancer.Conclusion: We think that prostate fusion biopsy can improve the pathologic results of patients by eliminating the potential sampling error of standard biopsy and with that may contribute to prostate cancer patient management
