Türkiye'de Streptococcus pyogenes suşlarında emm genotiplerinin dağılımı ve aşıyla uyumu
Gram-pozitif bakteriyel bir patojen olan Streptococcus pyogenes başta akut farenjit olmak üzere nekrotizan fasiit, yumu ?ak doku enfeksiyonlar? , glomerulonefrit, akut romatizmal ateş ve streptokokal toksik şok sendromu gibi ciddi hastalıklara neden olabilirler. Emm geni tarafından kodlanan M proteini, S. pyogenes suşlarının önemli bir virulans faktörüdür ve S. pyogenes e karşı oluşan antikor yanıtından büyük oranda sorumludur. S. pyogenes suşları emm genotipine göre sınıflandırılmaktadır. Günümüzde 124 tipi tanımlanmış olan emm geni dizisi, standart olarak DNA sekans analizi yöntemi ile belirlenmektedir. Ülkemizde Ankara, İstanbul, Zonguldak ve Konyada emm genotiplerinin dağılımı ile ilgili çalışmalar yapılmıştır. emm geni tiplendirilebilen toplam izolat sayısı 287 olan bu çalış- malara göre en sık saptanan genotipler; emm1 (%25,1), emm12 (%25,1), emm3 (%12,0), emm89 (%10,9), emm4 (%8,6) ve emm77 (%5,1) olarak görülmektedir. S. pyogenes için çok değerlikli aşı geliştirme çalışmaları yürütülmektedir. Türkiyede 26-valanlı aşının kapsama oranı %77,0dır (%74-%77,7). Dünyanın bazı bölgelerinde sık görülen emm tiplerinin a ?? kapsam ? nda olmamas ? ara ?t? r? c ? lar? yeni aşı geliştirmeye yöneltmiştir ve 30-valanlı olan bu yeni aşı da Türkiyede sık bulunan emm4 genotipi aşı kapsamına alınmış ve sonuç olarak kapsama oranı %91,9a çıkmıştır.
The distribution of emm genotypes in Streptococcus pyogenes strains and vaccine compliance in Turkey
Streptococcus pyogenes, a gram-positive bacterial pathogen, may cause particularly acute as well as necro- tizing fasciitis, soft tissue infections, glomerulonephritis, acute rheumatic fever and streptococcal toxic shock syndrome. M protein, encodedd by ??? gene, is an emergent virulence factor of ???????????? strains and is mostly responsible for the antibody response against this pathogen. ???????????? strains are classified according to ??? genotypes. ??? gene sequences of which 124 types have been defined to date are determined by DNA sequence analysis as a standard method. Studies in Ankara, Istanbul, Zonguldak and Konya were conducted on the distribution of ??? genotypes in Turkey. According to these studies in which a total of 287 isolates which could be typed, the most common genotypes were ??? 1 (25.1%), ??? 12 (25.1%), ??? 3 (12.0%), ??? 89 (10.9%), ??? 4 (8.6%) and ??? 77 (5.1%), respectively. Trials are currently conducted for developing mutli-valent vaccine. In Turkey, the coverage rate of 26-valent vaccine is 77,0% (74.0-77.7%). The uncovering the common ??? types in some regions of the world has led the researchers to develop new vaccines and the genotype ??? 4 which is common in Turkey isolates are included in this new 30- valent vaccine and in conclusion, the coverage rate has increased to 91.9%.
