Bir endemik guatr bölgesindeki 7 12 yaş grubu 73 sağlıklı ilkokul çocuğunda iyot ve selenyum düzeylerinin tiroid volümleri ve tiroid fonksiyonları üzerine olan etkileri araştırıldı Çocukların 38 i 52 kız 35 i 48 erkek olup ortalama yaş 9 56±1 77 yıl idi Çocukların 32 sinde 43 8 palpasyonda 56 sında 76 7 ultrasonografi ile guatr tespit edildi Ultrasonograf i ile tiroid volümleri 7 8 9 10 11 ve 12 yaşlar için sırasıyla 8 61±2 78 10 63±4 31 12 30±3 0 11 10±4 69 11 84±6 52 ve 18 94±12 52 ml olarak saptandı Ortalama T3 ve TSH düzeyleri normalin üst sınırlarında T4 düzeyleri normal sınırlarda triglobulin düzeyleri ise oldukça yüksek saptandı Ortalama serum selenyum düzeyi 30 84±23 04 956;g L ortalama idrar iyot düzeyi 3 91±3 77 956;g dl olup orta derecede selenyum ve iyot eksikliği olarak yorumlandı Tiroid volümlerinin yaş ağırlık boy ve kulaç uzunluğu ile pozitif serum selenyum düzeyi ise negatif korelasyon gösterdiği fakat idrar iyot düzeyi ve tiroid hormonları ile korelasyon bulunmadığı saptandı Bölgede iyot ve selenyum eksikliğine bağlı olarak önemli oranda guatr bulunduğu tiroid fonksiyonlarının olumsuz yönde etkilendiği iyot ve Se desteği yapılması gerektiği sonucuna varıldı Anahtar kelimeler: Endemik Guatr Okul Çocukları Tiroid Volümü İyot Selenyum

This study was carried out to establish the effects of iodine and selenium levels on thyroid gland size and thyroid functions in 73 healthy school children 7 12 year old mean 9 56±1 77 years 38 girls 52 and 35 boys 48 living in an endemic goiter area Goiter was found in 32 of the children 43 8 by palpation and 56 of the children 76 7 by ultrasonography The thyroid volumes were determined by ultrasound for the children at the ages of 7 8 9 10 11 and 12 years to be 8 61±2 78 ml 10 63±4 31 ml 12 30±3 0 ml 11 10±4 69 ml 14 84±6 52 ml and 18 94±12 52 ml respectively Mean serum T3 and TSH levels were in the upper limit of normal and mean serum T4 levels were within the normal limits but mean serum Tg levels were higher than the normal limits Mean serum Se levels were 30 84±23 04 956;g L mean urinary iodine levels were 3 91±3 77 956;g dl to be appropriate for the moderate iodine and Se deficiencies Thyroid volumes of the children were positively correlated with age body weight height arm span and were negatively correlated with serum Se levels but there was no correlation with urinary iodine levels and thyroid hormones As a result the school children in this area had significant goiter problems probably due to the iodine and Se deficiencies These problems effect the thyroid functions of the children negatively and immediately iodine prophylaxis and Se supplementation must be provided for this area Key words: Endemic Goiter School Children Thyroid Volumes Iodine Selenium
