BİLATERAL OVERYAN VE PSEUDOMYXOMA PERİTONEİ İLE BİRLİKTE YAYGIN KARIN İÇİ METASTAZLARA NEDEN OLAN APENDİKSTEN KAYNAKLANAN MÜSİNÖZ ADENOKANSER OLGUSU
Amaç: Apendiks adenokarsinomu tanısı alan, bilateral overlere ve intraabdominal kaviteye yaygın metastaz yapan, aynı zamanda pseudomyxoma
peritonei saptanan bir olguyu güncel lteratür eşliğinde tartışmayı amaçladık
Olgu Sunumu: İntraabdominal kitle saptanan hastaya uygulanan laparatomi sonucu yaygın karın içi metastazlar ve pseudomyxoma peritonei
bulguları saptandı. İntestinal sistem eksplorasyonunda apendiks üzerinde saptanan nodüler yapılar nedeniyle apendektomi sonrası
primer tümörün apendiks kökenli olduğu görüldü.
Tartışma: Apendiksten kaynaklanan adenokanser olgularında yaygın karın içi ve overyan metastazlar izlenebilmektedir. Bu durum primer
overyan bir maligniteyle karışabilmekte, sonuç olarak cerrahinin ve postoperatif adjuvan tedavinin şeklini etkileyebilmektedir. Bu yüzden
adneksiyal kitle saptanan hastalarda dikkatli bir intestinal sistem,özellikle apendiks, incelemesi yapılmalıdır.
A CASE REPORT OF MUSCINOUS ADENOCANCER ORIGINATED FROM APPENDIX WHICH CAUSED BILATERAL OVARIAN AND DIFFUSE INTRAABDOMINAL METASTASIS AND PSEUDOMYXOMA PERITONEI
Objective: We would like to discuss a case diagnosed as adenocarcinoma of appendix, which has a diffuse intrabdominal metastasis and
also has pseudomyxoma peritonei with the contemporary literature.
Case report: As the result of the laparatomy process performed to the patient prediagnosed with intraabdominal mass lesion, diffuse
metastasis and signs of pseudomyxoma peritonei. In exploration of gastrointestinal system, there has been nodular lesions determined
on the appendix and therefore appendectomy has been carried out. It has been seen that the primary tumor has been originated from
appendix.
Discussion: In cases with adenocarcinoma of appendix, diffuse intaabdominal and ovarian metastasis can be seen. This can make a confusion
about primary ovarian malignancies. As a result this issue can affect the surgical procedure and also the postoperative adjuvant
treatment modalities. That is why in patients with adnexial mass lesions, particular examination of intestinal system especially appendix
should be performed.
