Dev mediastinal kitlenin koil embolizasyonu sonrası tam rezeksiyonu: Olgu sunumu

Mediasten yerleşimli dev tümörlerin yerleşim yeriile tümörün kalp, büyük damar ve bronşiyal ağaçile komşulukları cerrahi tedavide önemlidir. Buyazıda ön mediasten yerleşimli dev tümöral kitleninvasküler invazyonu nedeni ile cerrahi tedavi öncesikoil embolizasyonunu takiben yapılan komplet cerrahirezeksiyonu sunuldu. Altı ay önce kanama nedeni ileyetersiz cerrahi rezeksiyon yapılmış olan teratom tanılıdev mediasten kitlesinin anjiyografisinde, iki taraflıinternal torasik arter (ITA) ve sağ lateral torasik arterdenbeslendiği saptandı. İki taraflı ITAya koil embolizasyonusonrasında total eksizyon yapıldı. Besleyici arterlerinanjiyografik olarak tespiti ve koil embolizasyon yapılarakgüvenli ve rezektabl kitle elde edilmesi mediastentümörlerin küratif tedavisini sağlamaya yardımcıdır.

Complete resection of a giant mediastinal mass following coil embolization: a case report

Location of mediastinal giant tumors and adjacency ofthe tumor to the heart, major vessels and bronchial treeare important for surgery. In this article, we present acomplete surgical resection following preoperative coilembolization for vascular invasion of anterior mediastinaloriginated giant tumor. Partially resected giant mediastinalmass due to bleeding was diagnosed as teratoma andwas found that the source of blood supply of tumor wasbilateral internal thoracic artery (ITA) and right lateralthoracic artery during angiography six months ago. Aftercoil embolization of bilateral ITA, complete excisionwas performed. Obtaining a safe and resectable mass bythe detection of vascular supply with angiography andapplication of coil embolization help to achieve curativetherapy of the mediastinal tumors.
