En Genç İleri Evre Grade III Endometrium Karsinomu: Olgu Sunumu
Giriş: Sıklıkla posmenapozal dönemde izlenen endometrium karsinomu en sık görülen jinekolojik malignensidir. Literatürde genç endometrial karsinom vakalarının; polikistik over hastalığı zemininde gelişen diferansiye erken evre kanserler olduğu vurgulanmaktadır. Bu makalede genç yaşta izlenen, ileri evre, kötü prognozlu endometrium karsinomu vakası sunulmuştur. Olgu Sunumu: Abortus sonrası revizyon küretaj yapılmasına rağmen vajinal kanaması devam eden 23 yaşındaki hastaya endometrial biopsi uygulanmış, patolojik inceleme sonucu Grade 3 adenokarsinom olarak gelmiştir. Yapılan operasyonda 8000 cc asit mai boşaltılmış, total abdominal histerektomi, bilateral salfingooferektomi, parsiyel omentektomi, bilateral pelvik-paraaortik lenf nodu diseksiyonu uygulanmıştır. Altı kür taksol-karboplatin kemoterapisi alan hastada operasyondan yaklaşık bir yıl sonra nüks gelişmiş, verilen kemoterapiye yanıt alınamamıştır. Hasta sepsis sebebiyle 24 yaşında kaybedilmiştir. Sonuç: Bu makalede endometriun kanseri, hiçbir risk faktörü saptanmayan 23 yaşındaki hastada izlenmiştir. Bu hasta literatürde bizim saptadığımız en genç ileri evre grade 3 endometrium kanseri olgusudur. Risk faktörleri dışında endometrium kanseri etyopatogenezinde rol alan mekanizmaları ortaya koyacak yeni yayınlara ihtiyaç vardır. Anahtar kelimeler: İleri Evre Endometrium Kanseri; En Genç.
The Youngest Advanced Stage Grade III Endometrial Carcinoma: A Case Report
Inroduction: Endometrail cancer, typically diagnosed in the postmenopausal years, is the most common gynecologic malignancy. Young women with endometrial carcinoma are considered to have a favorable prognosis with early stage, well differentiated lesions. In this report we present young patient with advanced stage endometrial adenocarcinoma who had poor prognosis. Case Report: A 23 year old female applied to the hospital because of postabortal bleeding despite revision curettage. Endometral sampling was perfomed and grade 3 endometrail adenocarcinoma was diagnosed. Athought total abdominal hysterectomy, bilateral salphingoopherectomy, pelvic-paraaortic lymphadenectomy, infracolic omentectomy and chemotherapy were performed, recurrence of the tumor was observed one year after the surgery. The patient died because of sepsis in 24 years old. Conclusion: In this report endometrial cancer was observed in 23 years old patient with no obvious risk factor. This report is the youngest advanced stage grade 3 endometrial adenocarcinoma in the literature. Further studies to evaluate the etyopathogenic mechanisms of endometrial cancer without risk factors are needed. Key words: Advanced Stage Endometrium Cancer; Youngest.
