Yanlışlıkla tüberküloz tanısı konulan 23 sarkoidoz olgusunun analizi
Sarkoidoz tanısı, klinik tablo ile uyumlu histopatolojik nonnekrotizan granülomların bulunması ve tüberküloz gibi benzer klinik tabloya yol açabilecek hastalıkların ekarte edilmesi ile konulur. Tüberküloz ve sarkoidozun klinik tabloları birbirlerini taklit edebilir, serum anjiotensin konverting enzim düzeyleri tüberkülozda yükselebilir veya sarkoidozda tüberkülin deri testi pozitif olarak bulunabilir ve bu nedenlerle özellikle tüberküloz prevalansının yüksek olduğu ülkelerde iki hastalığı ayırt etmek kolay değildir. Bu çalışmada sarkoidoz tanısı konulan ancak daha önce tüberküloz tedavisi alan hastaların dosyaları taranarak klinik ve radyolojik özellikler, tüberkülin deri testi pozitifliği, tüberküloz tedavisine başlanılma nedenleri ve sarkoidoz tanısı konulmasında kullanılan yöntemler gözden geçirildi. Tüberküloz tanısının büyük ölçüde klinik ve radyolojik özelliklere dayanarak konulduğu ve antitüberküloz tedaviye başlanılmasının en sık nedeninin pozitif tüberkülin testi olduğu saptandı.
Analysis of 23 cases of sarcoidosis misdiagnosed as tuberculosis
The diagnostic criteria for sarcoidosis require a compatible clinical picture, the histologic finding of noncaseating epitheloid tubercles, and a thorough evaluation to exclude other diseases such as tuberculosis which is capable of producing the same clinical picture. Clinical and radiological features of sarcoidosis and tuberculosis mimic each other, serum angiotensin converting enzyme levels can be increase in tuberculosis, and tuberculin skin test can be positive in sarcoidosis thus it is not easy to differentiate sarcoidosis from tuberculosis especially in countries where prevalance of tuberculosis is high. This study was undertaken to ascertain characteristics of patients with sarcoidosis who were diagnosed and treated as tuberculosis previously. The charts of all patients were reviewed for clinical features, radiografic features, tuberculin skin test anergy, the reason for antituberculosis treatment desicion and the method used to substantiate the diagnosis. It was found that the diagnosis of tuberculosis mostly depends on clinico-radiological features and the tuberculin skin test positivity was the commonest reason for antituberculosis treatment.
