Primer benign toraks duvarı tümörleri "103 olgunun retrospektif analizi"
Primer toraks duvarı tümörleri, malign ve benign olarak 2 başlık altında değerlendirilmektedir. 1985-2000 yılları arasında 103 hasta (52 erkek, 51 kadın) primer benign toraks duvarı tümörü tanısı ile opere edildi. Ortalama yaş 38.5 (8-65 yaş) yıldı. Hastaların %59.2'sinde göğüs duvarında şişlik, %27.2'sinde göğüs ağrısı, %26.2'sinde yan ağrısı mevcuttu. Fizik muayenelerinde; %83.4'ünde göğüs duvarında kitle, %9.7'sinde sternumda kitle saptandı. Radyolojik incelemede; %38.9'unda kemik yapılarda ekspansiyon, %14.6'sında kemikte litik lezyon saptandı. Hastaların 10'unda rekonstrüksiyon gerektirecek şekilde 66 olguda toraks duvarı rezeksiyonu, 35'ine ise kitle eksizyonu yapıldı. Histopatolojik incelemede; 28'inde osteokondrom, 21'inde kondrom, 11'inde fibröz displazi, 16'sında "schwannoma", 11'inde lipom, 3'er hastada hemanjiom, anjlofibrom, eozinofilik granülom, 2'şer hastada fibrom ve benign mezanşimal tümör, 1'er hastada desmoid tümör, lenfanjiyom, anevrizmal kemik kisti saptandı. Operatif mortalite yoktu, morbidite 2 hastada reeksplorasyon, 4 hastada yara yeri infeksiyonu ve 1 hastada solunum yetmezliği olmak üzere %6.8'di, hasta takipleri benign bir patolojide olmasının etkisi ile ancak hastaların yarısından azında gerçekleştirildi ve desmoid tümörlü hastada 4. yılda nüks dışında nüks saptanmadı. Primer benign toraks duvarı tümörleri semptomları, bulguları ve radyolojik görünümleri ile primer malign toraks duvarı tümörlerini taklit edebilmektedir. Bu nedenle cerrahi tedavi, tanı ve tedavi için uygun bir yöntemdir.
Primary benign chest wall tumors "Retrospective analysis of 103 cases"
Primary chest wall tumors are classified as benign and malignant. Between years 1985-2000, 103 patients (52 male and 51 female) were operated with the diagnosis of primary chest wall tumor. Mean age was 38.5 (8-65) years old. Symptoms were; swelling on the chest wall (59.2%), chest pain, (27.2%) and side-ache (26.2%). On physical examination, mass on the chest wall in 83.4% and sternal mass in 9.7% were determined. In radiologic examinations we determined expansion in 38.9% and litic lesions in 14.6% of the patients. Histopathological diagnoses were osteochondroma (28), chondroma (21), fibrous dysplasia (11), schwannoma (16), lipoma (11), hemangioma, angiofibroma and eosinophilic granuloma (three of each), fibroma and benign mesenchymal tumor (two of each) and desmoid tumor, lymphangioma, aneurysm one of each. No operative mortality was seen, morbidity rate was 6.8%; reexploration in two, wound infection in four and respiratuary insufficiency in one patient. Follow-up could be completed in lesser than half of the patients and no recurrency was seen except desmoid tumor patient 4 years after the operation. Primary benign chest wall tumors can imitate the malignant ones with symptoms, physical and radiological findings. For this reason surgical resection is suitable for the diagnosis and management.
