Küçük hücreli dışı akciğer kanserlerinde doku polipeptid antijen ( TPA ), CEA, CA 125, Alfa feto protein ( AFP )' in preoperatif değeri
Küçük hücreli dışı akciğer kanserlerinde (KHDAK) TPA, CEA, CA 125 ve AFP'in preoperatif, radyoterapi (RT) ve kemotera-pi(KT) öncesinde ölçüldüğünde hastalığın yaygınlığını ve operabütesini belirlemedeki değeri araştırıldı. Yetmişbeş KHDAK tanısı almış hastada eureleme için toraks BT, kemik sintigrafisi, abdominal (JSG üeyâCT, Kranial CT, bronkoskopi yapıldı. Tüm hastalarda tedavi öncesinde kan alınarak TPA, CEA, CA 125, AFP ölçüldü. Hastaların evreleri değerlendirildiğinde Ev-re l'de 10, Evre II' de 7, Evre IHA' da 10, Evre I1IB' de 26, Evre IV te 22 olgu vardı. Sadece 21 olgu operabldı. Diğerlerine RT ya da KT yapıldı. TPA, CEA, CA 125 histolojik tipden bağımsızdı. TPA metastaz olanlarda ve ileri evrelerde daha yüksek bulundu. CEA, CA 125 erken evreden ziyade genelde ileri evrelerde yüksek bulundu. Hastalar operabilite durumuna göre kesin operabl ( evre 1,11), muhtemel operabl ( evrelHA ) kesin inoperabl (evre IIIB; IV) gruplara aynldığında TPA kesin ope-rabl grupta daha düşüktü. Belli cut-off değerleri alındığında (TPA cut-of 100 U/L) kesin operabl grubu tahmin etmedeki spesifıtesi %71, sensitivitesi %70, pozitif prediktifdeğer (PPD) %44, negatif prediktifdeğer (NPD) %88 bulundu. Evre IHA ve Evre IIIB arasında anlamlı fark yoktu. TPA tümör yükü ile ilişkilidir. TPA yüksekliği kötü prognostik faktördür. Sonuç olarak; TPA operabiliteyi tahmin etmede kullanılabilir. Bu konuda altın standart radyolojik ve klinik değerlendirmedir. Ge-rekirse invaziv tanı yöntemleri planlanabilir. Bu aşamada yüksek değerlerde TPA olması inoperabilite hakkında fikir vere-bilir.
Preoperative value of TPA, CEA, CA 125 and AFP in nonsmall cell lung cancer
Preoperative Value of TPA, CEA, CA 125 And AFP in Nonsmall Cell Lung Cancer The preoperative value of TPA, CEA, CA 125 and AFP for determination of extensibility and operability before radiotherapy and chemotherapy in nonsmal cell lung cancers was studied. In seventy five cases, diagnosed as HSCLC, computed tomog-raphies of the thorax and the cranium, abdominal ultrasonography or tomography, bone scanning and broncoscopy were performed for staging of the disease. In all patients, the levels of TPA, CEA, CA 125 and AFP were measured in serum be-fore treatment. There were 10 patients in stage I, 7 in stage II, 10 in stage IHA, 26 in stage IIIB and 22 in stage IV. Only 21 ca-ses were operable. Radioterapy and chemotherapy were given to the others. TPA, CEA and Ca 125 were independent of histologic'types. While TPA was elevated in metastatic and late stages of the disease, CEA and Ca 125 were found to be ele-vated in late stages rather than early. When the patients were divided into groups of full operabl (stage 1,11), possible operable (stage IHA), and full inoperable (stage 1I1B, IV), TPA values were lower in definitely operable group. In evaluation with cut-off values ( TPA cut-off value was 100 m/L) specificity for estimation of operability was 71 %, sensitivity was 70%, posi-tive predictive value was 44%, and negative predictive value was 88%. There was no significant difference between stages IIIA and I1IB, TPA is associated with tumor mass. Elevated levels of TPA is a poor prognostic factor and can be used to pre-dict operability. But the gold standart of operability is still radiologic and clinical evaluation. When needed, invasive diag-nostic techniques can be performed . In this step, elevated levels of TPA in serum can give an opinion about operability.
