Kronik solunum yetmezlikli olgularda nazal intermittan pozitif basınçlı ventilasyon (NIPPV) etkinliği ve takipte karşılaşılan sorunlar
Kronik solunum yetmezlikli olgularda evde noninvaziv ventilasyon tedavisi nöromuskuler hastalık grubu dışında tartışmalıdır. Biz de bu çalışmada uzun süreli "bi-level positive airway pressure (BiPAP)" kullanır kararı verdiğimiz hastalarımızda tedavi etkinliğini ve tedaviyi sürdürmede karşılaşılan sıkıntıları irdelemeyi hedefledik. Kliniğimizde 2 yılda toplam 15 olguya kronik solunum yetmezliği tanısıyla uzun süreli noninvaziv mekanik ventilasyon kararı verilmiş olup, bu olgular daha öncesinde çeşitli kereler yatarak takip edilmiş olan hiperkapnik olgular veya stabil evrede BiPAP almadığı dönemde hiperkapni gelişmiş vakalardı. Bu olguların 2'sinde göğüs duvarı deformitesi, 6'sında amfizem, 1'inde tüberküloz sekeli ve 6'sında kronik bronşit mevcuttu. Bu olguların hepsi kor pulmonale gelişmiş olgular idi. Olguların BiPAP kararı verildiği dönemdeki ortalama pH: 7.37 ± 0.03, pCO2: 59.62 ± 5.24 mmHg, pO2: 43 ± 5.95 mmHg idi. Bu olguların üçü bağlı bulundukları kurumdan cihazı temin edememiş, iki olgunun karar verildiği dönemden cihaz teminine kadar birer kez yatmaları gerekmiş, 2 olgu 4. ayda, bir olgu 2 hafta sonra exitus olmuştur. Tüberküloz sekeli olan olgu 9 ay sonra, ağır amfizemi olan bir olgu ise 18 ay sonra exitus olmuştur. Beş olguda uyum problemi gözlenmiştir. En iyi uyum tb sekeli ve göğüs duvarı deformitesi olanlarda gözlenmiştir. Olgulara yapılan anket neticesinde efor dispnesi, uyku kalitesi, sabah baş ağrılarında belirgin düzelme olduğu saptanmıştır. Ortalama bir yıllık takip süresi içinde pH'nın korunduğu, hiperkapni ve hipokseminin kısmen düzeldiği gözlenmiştir. İlk bulgularımız koopere olgularda uzun süreli BiPAP kullanımının etkin olabileceğini düşündürmektedir.
Efficacy of noninvasive positive pressure ventilation in patients with chronic respiratory failure
Efficacy of noninvasive positive pressure ventilation in patients with chronic respiratory failure is a matter of debate except the neuromuscular patients. We aimed to evaluate the efficacy of this treatment modality and discuss the problems on follow-up of patients with chronic respiratory failure with various underlying diseases. 15 patients who were hospitalized several times due to chronic respiratory failure were decided to put on home mechanical ventilation programme (emphysema: 6, sequel of tuberculosis: 1, chronic bronchitis: 6, chest wall deformity: 2). All of these patients had cor pulmonale. Arterial blood gas analysis was as follows: pH: 7.37 ± 0.03, pCO2: 59.62 ± 5.24 mmHg, pO2: 43 ± 5.95 mmHg. Three of these cases could not get the machine; two cases had been hospitalized after the time of decision till the machine was available. Two cases died at the fourth month and one case died at the second week. A patient with the diagnosis of sequel of tuberculosis and another patient with severe emphysema had died 9 months and 18 months after the therapy respectively. Five patients were incompliant to the therapy. Compliance was better in the restrictive group. Results of questionnaire revealed improvement in sleep quality, exercise dyspnea and morning headaches. At the end of one-year follow-up, improvement in hypercapnia and hypoxemia was observed. The initial findings shows that long term NIPPV treatment can be an effective treatment modality in selected cases.
