İnvaziv aspergillozisin neden olduğu kosta destrüksiyonu (Olgu sunumu)
Aspergillozis Aspergillus, ailesine ait monomorfik mantarların oluşturduğu bir grup hastalığı ifade eder. İnfeksiyon sporların inhalasyonunu takiben pulmoner form olarak başlar, özellikle immün yetersizliği ve sistemik hastalığı olanlarda invaziv forma dönüşebilir. Altı aydır antitüberküloz tedavi alan bir yaşındaki kız hasta öksürük, ateş ve sol göğüste şişlik şikayetleriyle başvurdu. Akciğer grafisinde sol beşinci kostanın olmadığı görüldü. Kitleden ponksiyonla alınan apse kültüründe aspergillus üretilerek tanı sitopatolojik olarak doğrulandı. Radikal doku eksizyonu ve uygun antifungal tedaviyle iyileşerek taburcu edildi. Bilgilerimize göre bizim olgumuz, aspergillozisin neden olduğu kosta destrüksiyonunu gösteren ilk olgu olduğundan yayınlanmaya değer bulundu.
Destruction of rib due to invasive aspergillosis
Broadly defined, aspergillosis is a group of mycoses with monomorphic fungi of aspergillus species. Infection begins after the inhalation of spors in the pulmonary form and can progress into the invasive form especially in immunocomromise cases and also those with a systemic disease. A one year old girl who was on antituberculosis therapy for six monts admitted to our clinics with fever, cough and swelling on the left chest. On chest-x-ray, the left fifth costa was absent. Aspergillus grew on cultures which were obtained by puncture of the swelling, and diagnosis was cytopathogically supported. After radical excission of the mass and initiation of appropriate antifungal therapy, she was improved and discharged. To our knowledge, our case was considered worthreporting because it is the first that showed destruction of rib due to aspergillosis.
