Topiramat tedavisine yanıt veren primer öksürük baş ağrısı
Primer öksürük baş ağrısı ani başlangıçlı, öksürme, zorlanma ve/veya valsalva manevrası sonrası ortaya çıkan, bir saniye ile 30 dakika arasında sürebilen bir baş ağrısıdır. Sıklıkla 40 yaş üstü kişilerde ortaya çıkar ve bilateral yerleşimli keskin, saplanıcı bir ağrıdır. Yaklaşık %40 oranında organik bir neden bildirilmiştir. Bu nedenle tanı konmadan önce olası organik nedenler dışlanmalıdır. Tedavisinde karbonik anhidraz inhibitörleri ve intrakraniyal basıncı düşürebilecek ilaç ve uygulamalar kullanılmaktadır. Bu olguda zayıf karbonik anhidraz inhibisyonu yapmasından dolayı topiramatı tercih ettik ve tedavide oldukça etkili olduğunu gördük.
Primary cough headache responsive to topiramate
Primary cough headache strikes suddenly after coughing, straining and/or valsalva maneuver and lasts between one second to 30 minutes. It is most common among ages 40 and older and occurs bilaterally as a sharp, stabbing pain. Organic causes are reported to be about 40%. Therefore possible organic causes should be excluded before diagnosis. Treatment strategies include drugs like carbonic anhydrase inhibitors and procedures that may lower intracranial pressure. In this case we chose topiramate because of its weak carbonic anhydrase inhibition effect and found it effective for the treatment.
