An anomalous digestive arterial anastomosis connecting foregut, midgut and hindgut
Gastrointestinal yolun arteryel vaskülarizasyonu başlıca üç arterden oluşur; çölyak arter, superior ve inferior mezenterik arterler. Bu üç arter düzeyini birleştiren anastomozlar, aralarında Riolan, Villemin ve Bühler arkları ile Drummond'un marjinal arteri de bulunan çalışmalarla detaylı bir şekilde anlatılmıştır. Bunlar gerektiğinde etkin kolateral sirkülasyon sağlar ve dolayısıyla kolona cerrahi müdahale söz konusu olduğunda kullanılırlar. Bu olgu sunumunda, embriyolojik olarak açıklanabilen, dorsal pankreatik arterden başlayıp her üç arter düzeyini doğrudan birbirine bağlayan anastomotik kanal anlatıldı. Olgunun klinik önemi tartışıldı.
Ön barsak, orta barsak ve son barsağı birleştiren sıradışı arteryel anastomoz
The arterial vascularization of the gastrointestinal tract comprises mainly three arteries, the coeliac trunk, and both superior and inferior mesenteric arteries. The anastomoses joining these three levels of arteries are welldocumented and include arcades of Riolan, Villemin, Bühler, and marginal artery of Drummond. These provide efficient collateral circulation when needed and hence utilized for planning surgical intervention of colon. The present case describes an anastomotic channel, commencing from the dorsal pancreatic artery, and directly connecting all the three levels together that can be explained embryologically. The clinical significance of the present case has been discussed.
