Nazal kutanöz defektlerin rekonstrüksiyonunda supratroklear arter bazlı paramedyan alın flebi uygulaması

Amaç: Kutanöz burun defektlerinin rekonstrüksiyonunda kullandığımız paramedyan alın flebinin onkolojik ve fonksiyonel sonuçlarını değerlendirdik.Hastalar ve Yöntemler: Yassı hücreli veya bazalhücreli burun karsinomu nedeniyle ameliyat edilen14 olgunun rekonstrüksiyonunda supratroklear arter bazlı paramedyan alın flebi uygulandı. Hastalara sadece kitle eksizyonu yapılırken, lenf nodu diseksiyonu,radyoterapi veya kemoterapi uygulanmadı. Ortalama izlem süresi 42.4 ay dağılım 8-83 ay idi.Bulgular: Hiçbir olguda flep nekrozu, lokal nüks veya uzak metastaz görülmedi. On hastada %71.4 'değiflik oranlarda nefes alma güçlüğü gözlendi. Buflikayetin, flebin kontrakte olmasıyla zaman içindeazaldığı görüldü.Sonuç: Paramedyan alın flebi ile rekonstrüksiyondakabul edilebilir fonksiyonel ve baflarılı onkolojik sonuçlar elde edilmektedir

Reconstruction of nasal cutaneous defects with the use of supratrochlear artery-based paramedian forehead flap

Objectives: We evaluated oncologic and functional results of paramedian forehead flap in the recon­ struction of nasal cutaneous defects.Patients and Methods: Supratrochlear artery-based paramedian forehead flap was employed in 14 patients 13 men, 1 woman; mean age 57 years; range 46 to 63 years for the reconstruction of nasal cutaneous defects resulting from excision of squamous or basal celi carci- noma. No other treatment modalities were performed other than removal of the primary lesion. The mean fol- low-up was 42.4 months range 8 to 83 months .Results: None of the patients developed flap necrosis, local recurrences, or distant metastasis. Airway problems of varying extent were observed in 10 patients 71.4% , the severity of which became atten- uated in time as the flap gained proper contraction.Conclusion: Acceptable functional and successful oncologic results can be obtained in the reconstruc­ tion with the use of paramedian forehead flap.
