Dural arteriovenöz fistül saptanan iki olguda öncü belirti olarak pulsatil tinnitus

Normal otoskopik bulguların eşlik ettiği pulsatil tinni- tus, gerçekte transvers veya sigmoid sinüs dural ar- teriovenöz fistül TSDAVF gibi çok tehlikeli ve yaşa- mı tehdit edici özellik taşıyan bir hastalığın tek belir- tisi olabilmektedir. Tedavi edilmezse, TSDAVF’ler fo- kal nörolojik bulgulara, intrakranyal hipertansiyon veya kanamaya ve nihayet ölüme yol açabilirler. Bu yazıda, pulsatil tinnitus yakınması ile başvuran iki ol- gu sunuldu. İki olguya da TSDAVF tanısı konduktan sonra transarteryel embolizasyon uygulandı. Tedavi- den sonra olguların yakınmaları kayboldu

Pulsatile tinnitus as the presenting symptom of dural arteriovenous fistula in two cases

Pulsatile tinnitus associated with normal otoscopic findings may present as the only sign of a potential- ly life-threatening serious disease such as a dural arteriovenous fistula dAVF of the transverse or sig­ moid sinüs. If left untreated, dAVFs may lead to focal neurologic symptoms, intracranial hypertension, or hemorrhage, and finally to death. We report two cases of dAVF presenting with pulsatile tinnitus.Following diagnosis, both patients underwent transarterial embolization, after which their symp­ toms resolved.
