Kemik iliği ve kök hücre transplantasyon ilkeleri

Hematopoetik kök hücre transplantasyonu aplastik anemi, hematolojik malignensiler, doğumsal hematolojik defektler, immün yetmezlikler, kalıtsal hastalıklar ve solid tümörler gibi birçok hastalık için önemli bir tedavi yaklaşımıdır. Hematopoetik kök hücre transplantasyonuna hazırlanma, kemoterapi ve radyoterapi ile normal ve anormal hematopoezin baskılanmasını, verici hücrelerinin yapılanmasını sağlamak üzere etkin immünosupresyonu içermektedir. İmmün sistemi baskılanmış hastalarda fırsatçı mikroorganizmalar başta olmak üzere diğer enfeksiyonlar, verici hücrelerinin reddedilmesi, graft versus host hastalığı, primer hastalığın yinelenmesi gibi önemli ve mortalitesi yüksek komplikasyonlar gelişebilmektedir. Bu komplikasyonlar hematopoetik kök hücre transplantasyon başarısını sınırlamakta ise de son yıllarda transplantasyonla kurtulan hasta sayısının artmasıyla, tedavinin etkinliği de artmaktadır.

Principles of bone marrow and stem cell transplantation

Hematopoietic stem cell transplantation (HSCT) has become an important therapeutic modality for a wide variety of diseases including hematologic malignancies, aplastic anemia, immunodeficiency disorders, congenital hematologic defects, hereditary syndromes and solid tumors. Preparation for hematopoietic stem cell transplantation includes chemotherapy and radiotherapy to ablate normal and abnormal hematopoiesis and to provide immunosuppression to allow donor cell engraftment. Immunocompromised patients may suffer from important complications with high mortality, such as rejection of donor cells, graft versus host disease, recurrence of the primary disease and especially opportunistic and other types of infectios. Although these complications limit the success of HSCT, recently, the efficacy of the treatment has improved as there has been an increase in the number of patients surviving as a result of transplantations.
