SAFRA YOLLARI PATOLOJİLERİNDE ÇOK KESİTLİ BİLGİSAYARLI TOMOGRAFİ VE ENDOSKOPİK KOLANJİOPANKREATOGRAFİ BULGULARININ KARŞILAŞTIRILMASI

Amaç: Çalışmamızın amacı, safra yolu patolojisidüşünülen olgularda Multiplanar reformasyon(MPR) ve minimum intensite projeksiyon (MinIP)tekniklerini kullanarak çok kesitli bigisayarlıtomografi (ÇKBT) bulgularını altın standart olanendoskopik retrograd kolanjiopankreatografi(ERKP) ile karşılaştırarak ÇKBT’nin tanısaldeğerini araştırmaktır.Gereç ve Yöntemler: Çalışmaya laboratuar veklinik bulguları ile safra yolu patolojisi düşünülenve rutin çekimlerde safra yolu patolojisi tespitedilen 30 olguya ÇKBT çekimi sonrası ERKPyapıldı. MPR ve MinIP tekniklerinde safrayolarının ölçümleri yapıldı. ÇKBT’nin, MPR veMinIP teknikleriyle birlikte patoloji tespitinde,ERKP ile karşılaştırıldığında, pozitif kestirimdeğeri, negatif kestirim değeri, özgüllük veduyarlılığı hesaplandı.Bulgular Patoloji varlığı tespitindeki pozitifkestirim değeri %96.15, negatif kestirim değeri%25, özgüllüğü %50, duyarlılığı %89.28 olaraktespit edildi. ÇKBT’nin obstrüksiyon seviyesinitespiti ERKP ile karşılaştırıldığında %100 uyumluidi.Sonuç: Çok kesitli bilgisayarlı tomografi, MPR veMinIP tekniklerin gelişimiyle pankreatobiliyersistemin değerlendirilmesinde tanısal ERKP’ninyerini alabilecek noninvaziv bir yöntemdir. Ayrıcadiğer batın içi organların değerlendirilmesi vepankreatobiliyer tümörü bulunan hastaların,preoperatif tanısı ve evrelemesinde geniş kapsamlıbilgiler sunmaktadır.

The Comparıson Of Fındıngs Of Multı-Dedector Computed Tomography And Endoscopıc Cholangıopancreatography In Bıle Duct Pathologıes

Aim: To investigate diagnostic value of multidedector computed tomography (MDCT) usingmultiplanar reformation (MPR) and minimumintensity projection (MinIP) techniques withcomparison of MDCT findings and endoscopicretrograde cholangiopancreatography (ERCP)findings known as the “gold standart” in patientswith suspected biliary tract pathology.Material and Methods: 30 patients withconsidered biliary tract pathology according tolaboratory and clinical findings, and biliary tractpathology identified after routine imagings wereunderwent ERCP after MDCT included in thisstudy. Biliary tract measurments were made inMPR and MinIP. When MDCT using with MPRand MinIP techniques in the pathology detectionwas compared wtih ERCP, the pozitive predictivevalue, then negative predictive value, specifity andsensivity were calculated.Results: The pozitive predictive value, thennegative predictive value, specifity and sensivitywere found in the presence of patholgy detection as96.15%, 25%, 50%, 89.28%, respectively. Whencompared with ERCP, MDCT was 100%compatible for the detection of obstruction level.Conclusion: Multi-dedector computed tomographywith the development of MPR and MinIPtechniques is a noninvasive method to replacediagnostic ERCP. In addition, MDCT offerscomrehensive informations about other intraabdominal organs and preoperative diagnosis andstaging of patients with pancreatobiliary tumors
