Multiple Sklerozda Periferik Sinir Sistemi Tutulumunun Araştırılması
Multipl sklerozda (MS) santral sinir sistemi tutulması ön planda olduğu için perferik sinir iletimi çalışmalarında herhangi bir anormallik beklenmez. Ancak literatürde MS'li hastalarda periferik sinir iletim çalışmaları anormallikleri destekleyen çalışmalarda vardır. Bizim bu çalışmadaki amacımız Relapsing-Remitting formdaki MS'li (RRMS) hastalarda periferik sinir tutulumunun olup olmadığını, periferik sinir iletim çalışmaları ile ortaya koymaktır. 2001 yılında revize edilen McDonalds kriterlerine göre kesin RRMS tanısı alan 20 kişilik hasta grubu ile 15 kişilik kontrol grubu arasında yapılan elektromiyografi (EMG) ile median, peroneal, tibial ve sural sinirlerde ileti hızı, distal latans (DL) ve aksiyon potansiyeli amplitüt değerleri karşılaştırılması yapıldı. Hasta ve kontrol grubunda arasında yapılan EMG çalışmalarının karşılaştırılmasında ileti hızı, DL, aksiyon potansiyeli amplitütleri arasında istatiksel olarak anlamlı fark bulunamamıştır. Bizim yaptığımız sinir ileti çalışmalarında MS’li hastalarla, kontrol gurubu arasında istatiksel olarak anlamlı fark bulunamamış olmasına rağmen, çeşitli çalışmalarda MS’li hasta alt gruplarında yaklaşık %5 ile %14 arasında periferik sinir tutulumu gözlenmiştir. Bu konuda daha geniş örneklem büyüklüğüyle ve daha kapsamlı yapılacak çalışmalara ihtiyaç vardır.
Investigation of Peripheral Nerve Involvement in Multiple Sclerosis
Since central nervous system involvement is prominent in multiple sclerosis (MS), no abnormality is expected in peripheral nerve conduction studies. However, there are studies in the literature supporting the abnormalities in peripheral nerve conduction studies in patients with MS. Our aim in this study is to determine whether there is peripheral nerve involvement in patients with Relapsing-Remitting MS (RRMS) with peripheral nerve conduction studies. Conduction velocity, distal latency (DL) and action potential amplitude in median, peroneal, tibial and sural nerves were compared by electromyography (EMG) between 20 patients who were diagnosed with definite RRMS according to McDonalds criteria revised in 2001 and 15 control group. In the comparison of EMG studies performed between the patient and control groups, no statistically significant difference was found between conduction velocity, DL, and action potential amplitudes. Although no statistically significant difference was found between the patients with MS and the control group in our nerve conduction studies, peripheral nerve involvement was observed between 5% and 14% in the subgroups of patients with MS in various studies. There is a need for more comprehensive studies with a larger sample size on this subject.
