Parsiyel ve tam kat rotator manşon yırtıklarında artroskopik cerrahinin etkinliğinin subakromiyal sıkışma sendromu tedavisindeki kadar başarılı olup olmadığının karşılaştırılması
Amaç: Bu çalışmanın amacı parsiyel ve tam kat rotator manşon
yırtıklarının artroskopik tedavilerinin subakromiyal sıkışma sendromu
tedavisindeki kadar başarılı olup olmadığını değerlendirmektir.
Hastalar ve Yöntemler: Ocak 2010–Ocak 2012 yılları arasında
artroskopik olarak opere edilen 43 hasta (29 Kadın, 14 Erkek;
ortalama yaş 46,3±11,3) çalışmaya alındı. Subakromiyal sıkışma
sendromu tanısı olan hastalara artroskopik olarak subakromiyal
dekompresyon ve akromiyoplasti uygulandı. Rotator manşon yırtığı
tanısı olan hastalarda ise artroskopik subakromiyal dekompresyon
ve akromiyoplastiye ek olarak kısmi yırtıklarda yırtık bölgesinin
artroskopik debridmanı, tam kat yırtıklarda artroskopik olarak
suture anchorlar yardımı ile onarım uygulandı. Hastalar ConstantMurley omuz skorlaması ve aktif omuz eklem hareketler açıklığına
göre değerlendirildiler.
Bulgular: Ortalama takip süresi 13,5±2,7 aydı. Tüm grupların
pre-operatif Constant-Murley omuz skorları arasında istatistiksel
açıdan anlamlı bir fark bulunamadı. Son kontrollerdeki ConstantMurley omuz skoru ve aktif omuz eklem hareket açıklığı preoperatif değerlere göre subakromiyal sıkışma sendromlu, tam kat
rotator manşon yırtıklı ve <%50 kısmi kalınlıkta rotator manşon
yırtığı olan hastalarda anlamlı derecede iyi bulundu. Fakat >%50
kısmi kalınlıkta rotator manşon yırtığı olan hastalarda ConstantMurley omuz skorlarında ve aktif omuz eklem hareket açıklıklarında
istatistiksel olarak anlamlı derecede iyileşme görülmedi.
Sonuç: Subakromiyal sıkışma sendromu ve <%50 kısmi
kalınlıkta rotator manşon yırtığı tedavisinde artroskopik
subakromiyal dekompresyon ve akromiyoplasti başarılı sonuçlar
verirken >%50 kısmi kalınlıkta rotator manşon yırtıklarında başarılı
sonuçlar vermemektedir. Tam kat rotator manşon yırtıklarının
tedavisinde artroskopik onarımla akromiyoplastinin beraber
uygulanması başarılı sonuçlar verir.
Comparison of results of arthroscopic treatment for partial and total thickness tears of the rotator cuff with results of arthroscopic treatment for the subacromial impingement syndrome
Objectives: The aim of our study was to determine whether
results of arthroscopic treatment for partial and total thickness
rotator cuff tears are as successful as results of arthroscopic
treatment for subacromial impingement syndrome.
Patients and Methods: Forty-three patients (29 women,14
men;mean age 46.3±11.3 years) operated arthroscopically were
recruited to the study between January 2010 and January 2012.
Subacromial decompression and acromioplasty were used for the
subacromial impingement syndrome. In addition to that surgical
procedure, a debridement was used for the partial-thickness tear of
the rotator cuff, and a repair was made with suture-anchors for the
full-thickness tear of the rotator cuff. The patients were evaluated
according to the Constant-Murley score and on the basis of active
shoulder motions.
Results: Mean follow-up was 13.5±2.7 months.No significant
difference was found between all groups with regard to the preoperative Constant-Murley score.The final Constant-Murley score
and the active shoulder motions were significantly better than preoperative results in the subacromial impingement syndrome, fullthickness and <50%partial-thickness tears of the rotator cuff.
However, the improvement was not found in partial tears (>50%
of the thickness) of the rotator cuff .
Conclusion: Arthroscopic subacromial decompression and
acromioplasty give successful results for subacromial impingements
and for partial tears (<50% of the thicknes) of the rotator cuff.
However, that treatment does not give a successful result for tears
>50% of the thickness of the rotator cuff.The arthroscopic repair of
a tear of the rotator cuff and acromioplasty give successful results
of tears of the full-thickness of the rotator cuff.
